Summary: This study found that plasma neurofilament light (NfL) concentrations are higher in cognitively unimpaired older adults with the APOE epsilon 4 allele, which is the major genetic risk factor for Alzheimer's disease (AD). Plasma NfL concentrations were also significantly associated with AD-related neurodegeneration, amyloid-beta plaque burden, tau tangle burden, and memory scores.

Time to diagnosis in younger-onset dementia and the impact of a specialist diagnostic service

Summary: This study identified predictors of delay to diagnosis in people with younger-onset dementia (YOD) and found that younger age at onset, dementia type other than Alzheimer's disease (AD) and behavioral-variant frontotemporal dementia (bvFTD), and increased number of services consulted were significant predictors of diagnostic delay. Additionally, the study showed that a specialist YOD service can reduce time to diagnosis, highlighting the importance of such service in reducing diagnostic delay and providing post-diagnostic support.

Cerebrospinal fluid neurofilament light chain differentiates primary psychiatric disorders from rapidly progressive, Alzheimer's disease and frontotemporal disorders in clinical settings

Summary: This study investigated the clinical value of cerebrospinal fluid (CSF) neurofilament light (NfL) and total-tau (t-tau) in differentiating neurodegenerative disorders (ND) from psychiatric disorders (PSY) and rapidly progressive disorders. The results showed that NfL had a high accuracy in distinguishing ND from PSY, outperforming t-tau in most clinical scenarios. However, it was not effective in distinguishing Creutzfeldt-Jakob disease (CJD) from rapidly progressive dementias/atypically rapid variants of common ND (RapidND).

Summary: This study evaluated the performance of neurofilament light chain (NfL) testing in diagnosing anti-NMDA receptor encephalitis (NMDARe) and first episode of psychosis (FEP) caused by a psychiatric disease. The results showed that NfL levels were associated with disease severity in NMDARe but not with long-term outcome. FEP patients with NfL levels ≥15 pg/mL had a significantly higher chance of having NMDARe.

Neurofilaments light: Possible biomarker of brain modifications in bipolar disorder

The study analyzed data from the National Health and Nutrition Examination Survey (NHANES) and found that age, creatinine, and glycosylated hemoglobin (HbA1c) were the best predictors of sNfL levels. The results provide guidance for interpreting sNfL levels in relation to neurologic conditions.
