Summary: Diagnosing angina pectoris and dyspnea in patients with normal or nonobstructive coronary vessels is challenging. Invasive coronary angiography can identify nonobstructive coronary artery disease, but coronary microvascular dysfunction may actually be the cause of symptoms. Positron emission tomography (PET) allows noninvasive detection and delineation of coronary microvascular dysfunction by quantifying myocardial blood flow at rest and during vasodilation. Individualized or intensified medical therapies may improve symptoms, quality of life, and outcomes. Standardized diagnosis and reporting criteria for ischemic symptoms caused by coronary microvascular dysfunction are crucial for treatment decisions. A consensus document proposes standardized diagnosis, nomenclature, and reporting criteria for coronary microvascular dysfunction. It aims to provide an overview of its pathophysiology, assessment, and classification, which are essential for diagnosis, patient care, and clinical trials.
