Addressing Microbial Resistance Worldwide: Challenges over Controlling Life-Threatening Fungal Infections

Leonardo Martins-Santana et al.

Summary: Fungal infections, especially those caused by drug-resistant fungi, pose a serious global health concern. Candida, Aspergillus, and Cryptococcus are some of the fungi that cause severe and systemic infections, with high mortality rates. Understanding the immunopathogenesis of fungal diseases and finding ways to prevent dissemination and deaths caused by fungal infections are crucial. This review highlights the importance of drug resistance in clinically significant fungi and discusses strategies to combat these infections.

Antifungal drug resistance: an update

David W. Denning

Summary: The limited number of antifungal classes and increasing resistance highlight the need for more focus on susceptibility testing and antifungal stewardship, which have proven to improve survival and overall clinical outcomes. Positive diagnosis of fungal infections through laboratory markers can allow for discontinuation of non-specific antibacterial therapy, contributing to antimicrobial resistance control. Non-culture based diagnostics for fungal diseases offer great sensitivity and speed, but are less effective in identifying antifungal resistance.

Recent advances in metabolomics analysis for early drug development

Juan Carlos Alarcon-Barrera et al.

Summary: Metabolomics has now become an accepted and widely applied approach in early drug development in the pharmaceutical industry. It has evolved from a pure exploratory tool to a more mature and quantitative biochemical technology. Metabolomics analysis helps in defining physiological response, target engagement markers, and elucidating the mode of action of drug candidates under investigation.

Tackling the emerging threat of antifungal resistance to human health

Matthew C. Fisher et al.

Summary: This review emphasizes the important threat of invasive fungal infections to public health and the emergence of antifungal resistance. It discusses the main mechanisms of antifungal resistance and compares them with bacterial resistance to antimicrobial control. The review also explores the research and innovation topics needed to minimize the occurrence and impact of antifungal resistance, including environmental links, surveillance, diagnostics, transmission routes, novel therapeutics, and hotspot mitigation strategies. The need for global efforts to manage existing antifungal drugs and direct future research and development of therapies and interventions is highlighted.

The Role of Diagnostics-Driven Antifungal Stewardship in the Management of Invasive Fungal Infections: A Systematic Literature Review

Arunaloke Chakrabarti et al.

Summary: Antifungal stewardship (AFS) programs are crucial for optimizing antifungal use and improving outcomes in patients with invasive fungal infections. Our systematic literature review evaluates the impact of diagnostics in AFS programs. The review finds that diagnostic-driven interventions can significantly reduce species identification time, time to targeted therapy, antifungal consumption, cost, mortality rate, and length of stay in patients with invasive fungal infections.

Molecular investigations on Candida glabrata clinical isolates for pharmacological targeting

Hrishikesh Gupta et al.

Summary: The prevalence of drug resistant C. glabrata strains in immune-compromised patients has increased rapidly. To identify the mutations/variations and update antifungal therapy, researchers comprehensively characterized the clinical isolates using molecular and metabolic profiling methods. Significant variations in susceptibility, surface hydrophobicity, and oxidative stress generation were observed. Metabolic profiling revealed differential metabolites, while transcriptional analysis identified major differences in cellular pathways related to stress, metabolism, and biosynthesis. This study provides valuable information for differentiating pathogenic isolates from wild-type and understanding their enhanced pathogenicity.
