Redefining Staphylococcus aureus bacteremia: A structured approach guiding diagnostic and therapeutic management

Summary: The current approach to categorizing patients with Staphylococcus aureus bacteremia (SAB) into complicated and uncomplicated disease fails to consider the heterogeneity of SAB. This article proposes a two-step system to categorize SAB patients as "high risk" or "low risk" for metastatic infection and suggests additional diagnostic evaluation for high-risk patients to inform an individualized treatment plan.

Summary: Antistaphylococcal penicillins (ASP) and cefazolin are the first-line treatment options for methicillin-susceptible Staphylococcus aureus (MSSA) bacteremia. Strains with borderline oxacillin resistance due to hyperproducing beta-lactamases pose a challenge. This study found that the oxacillin MIC did not affect the acute treatment outcomes of MSSA bacteremia patients, regardless of the beta-lactam selected as definitive therapy.
