Using the ACMG/AMP framework to capture evidence related to predicted and observed impact on splicing: Recommendations from the ClinGen SVI Splicing Subgroup

Logan C. Walker et al.

Summary: The ACMG/AMP framework for classifying variants related to splicing potential uses six evidence categories. However, the lack of guidance on code application has led to variation in specifications developed by different panels. The ClinGen Splicing Subgroup was established to refine recommendations for applying ACMG/AMP codes. Recommendations and approaches for evaluating RNA-assay evidence aim to standardize variant classification processes based on splicing-based evidence.

The performance of genome sequencing as a first-tier test for neurodevelopmental disorders

Bart P. G. H. van der Sanden et al.

Summary: Genome sequencing (GS) is a technique that can provide novel diagnoses for patients who remain undiagnosed. In a study involving patients with neurodevelopmental disorders, GS and standard of care (SOC) had similar diagnostic yield, but GS identified two additional conclusive diagnoses and four additional possible diagnoses. GS comprehensively identified all variants in a single experiment, suggesting it is a more efficient genetic diagnostic workflow.

Genetic and metabolic investigations for neurodevelopmental disorders: position statement of the Canadian College of Medical Geneticists (CCMG)

Melissa T. Carter et al.

Summary: This article provides recommendations for clinicians regarding the use of genetic and metabolic investigations for patients with neurodevelopmental disorders (NDDs), such as global developmental delay (GDD), intellectual disability (ID), and/or autism spectrum disorder (ASD). The recommended tests include chromosomal microarray and fragile X testing as first-tier tests, metabolic investigations for clinical features suggestive of inherited metabolic disease, and exome sequencing or comprehensive gene panel as a second-tier test for GDD or ID patients. Genetic testing is not recommended for NDD patients without GDD, ID, or ASD, unless there are clinical features of syndromic etiology or inherited metabolic disease.

Diagnostic yield of pediatric and prenatal exome sequencing in a diverse population

Anne Slavotinek et al.

Summary: This study evaluated the diagnostic yield of exome sequencing (ES) in individuals of European and underrepresented minority (URM) and underserved (US) patients. The results showed a higher diagnostic rate in pediatric patients compared to prenatal patients, but there was no significant difference in diagnostic yield or inconclusive findings between URM/non-URM and US/non-US patients. These findings support the use of ES to identify clinically relevant variants in patients from diverse populations.

Diagnostic yield of genome sequencing for prenatal diagnosis of fetal structural anomalies

Yiming Wang et al.

Summary: Genome sequencing has the potential to be a comprehensive prenatal test for the diagnosis of fetal abnormalities. In this study, sequencing identified diagnostic variants in 14% of fetuses and variants of uncertain significance in 19% of fetuses. Genome sequencing offers a more comprehensive analysis and has the potential to replace multiple consecutive tests.

Exploring the diagnostic utility of genome sequencing for fetal congenital heart defects

Y. Cao et al.

Summary: This pilot study investigated the utility of genome sequencing for prenatal diagnosis of congenital heart defects (CHD). The results showed that genome sequencing expands the diagnostic scope of mutation types and reveals the genetic etiology for fetal heart anomalies. Some patients may choose to continue the pregnancy, especially if the heart defect can be surgically repaired.

A complete reference genome improves analysis of human genetic variation

Sergey Aganezov et al.

Summary: Compared to its predecessors, the Telomere-to-Telomere CHM13 genome has significant improvements in sequence length and structural accuracy, enabling more comprehensive study of the human genome. The application of the T2T-CHM13 reference has demonstrated improved accuracy in read mapping and variant calling for globally diverse samples, leading to the discovery of previously unresolved variants and the removal of false positives. These advancements position T2T-CHM13 as a potential replacement for GRCh38 as the dominant reference for human genetics.

Diagnostic yield using whole-genome sequencing and in-silico panel of 281 genes associated with non-immune hydrops fetalis in clinical setting

E. Westenius et al.

Summary: The objective of this study was to evaluate the diagnostic yield of clinical whole-genome sequencing (WGS) in prenatal diagnosis of non-immune hydrops fetalis (NIHF). The results showed that 52% of the fetuses in the study achieved a molecular diagnosis through WGS. However, caution is needed in interpreting the results due to the small sample size and possible over-representation of severe phenotypes.

Calibration of computational tools for missense variant pathogenicity classification and ClinGen recommendations for PP3/BP4 criteria

Vikas Pejaver et al.

Summary: The American College of Medical Genetics and Genomics and the Association for Molecular Pathology (ACMG/AMP) provide recommendations for interpreting sequence variants, specifying the use of computational predictors as evidence for pathogenicity or benignity. However, the lack of quantitative support in the score intervals defined by tool developers and ACMG/AMP recommendations that require consensus of multiple predictors is addressed by a probabilistic framework proposed by the researchers. This framework is extended to computational predictors and introduces a new standard for converting tool scores to evidence strengths.
