Atrial Fibrillation (AFIB) in the ICU: Incidence, Risk Factors, and Outcomes: The International AFIB-ICU Cohort Study*

Summary: This study aimed to assess the incidence, risk factors, and outcomes of atrial fibrillation (AF) in ICU and described the current practice in the management of AF. The results showed that the incidence of AF was 15.6%, with newly developed AF accounting for 13.3%. Factors associated with AF included arterial hypertension, paroxysmal AF, sepsis, or high disease severity at ICU admission. There were variations in the diagnostic and management strategies for AF, and AF was associated with worse outcomes.

Impact of Lactate on 30-Day All-Cause Mortality in Patients with and without Out-of-Hospital Cardiac Arrest Due to Cardiogenic Shock

Summary: In patients with cardiogenic shock (CS) due to myocardial infarction, elevated lactate levels are negative predictors. The prognostic impact in patients with CS complicated by out-of-hospital cardiac arrest (OHCA) is limited. This study found that lactate values were associated with 30-day all-cause mortality in both the non-OHCA and OHCA groups. Particularly, lactate levels on days 1 and 8 were more accurate in predicting all-cause mortality in CS patients with OHCA.

The risk of stroke may be higher in newly diagnosed atrial fibrillation compared to prior atrial fibrillation.
