Advances in contemporary medical management to treat patients with heart failure

Summary: Neurohormonal blockade and device-based therapies are effective in reducing morbidity and mortality in heart failure. SGLT2 inhibitors have shown benefits in various heart failure phenotypes. In addition to SGLT2 inhibitors, drugs targeting novel pathways have demonstrated benefit in heart failure with reduced ejection fraction. This review provides an update on recent advances in the medical management of heart failure, including the role of SGLT2 inhibitors, vericiguat, and omecamtiv mecarbil.

Summary: In recent two decades, there has been an increasing use of bioprostheses in surgical aortic valve replacement, despite the known process of structural valve deterioration (SVD). The long-term durability of surgical bioprostheses is still not fully understood, as it has been extensively evaluated with various definitions and methods. With the widespread adoption of transcatheter aortic valve implantation (TAVI) technology, understanding the long-term durability of transcatheter bioprostheses has become increasingly important, although limited data is available compared to surgical experience. This lack of knowledge is a major barrier to the use of TAVI platforms for younger patients.

Transcatheter aortic valve implantation and frailty

Summary: This review discusses important frailty assessment tools that can be used as prognostic factors for patients before transcatheter aortic valve implantation (TAVI). The tools were categorized into different types and further stratified based on their use. The Clinical Frailty Scale (CFS) is a semi-quantitative tool that can be easily used in outpatient settings. Serum albumin levels were considered as qualitative assessment markers. The Essential Frailty Toolset showed high reliability in predicting mortality. Some studies also focused on changes in frailty pre- and post-TAVI. Therefore, the selection of appropriate frailty assessment tools should consider the setting and patient's condition.
