Combining loop with thiazide diuretics for decompensated heart failure: the CLOROTIC trial

Joan Carles Trullas et al.

Summary: This study aims to evaluate whether adding hydrochlorothiazide (HCTZ) to intravenous furosemide is a safe and effective strategy for improving diuretic response in acute heart failure (AHF). The study found that adding HCTZ to loop diuretic therapy can improve diuretic response in patients with AHF, but may also lead to renal function impairment.

The win ratio method in heart failure trials: lessons learnt from EMPULSE

Stuart J. Pocock et al.

Summary: The EMPULSE trial compared empagliflozin with placebo in patients with acute heart failure to evaluate the clinical benefit. The study found that using the win ratio analysis strategy can integrate deaths, clinical events, and patient-reported outcomes to provide clinically meaningful findings of patient benefit.

Natriuresis-guided therapy in acute heart failure: rationale and design of the Pragmatic Urinary Sodium-based treatment algoritHm in Acute Heart Failure (PUSH-AHF) trial

Jozine M. ter Maaten et al.

Summary: This study aims to investigate whether natriuresis-guided therapy improves natriuresis and clinical outcomes in patients with acute HF. The study will adjust the treatment based on the urinary sodium levels in the natriuresis-guided group using a pre-specified stepwise approach, aiming to determine the efficacy of this treatment method in improving patients' natriuresis.

Renal profiling based on estimated glomerular filtration rate and spot urine sodium identifies high-risk acute heart failure patients

Jan Biegus et al.

Summary: In patients with acute heart failure, there is no correlation between spot urine sodium concentration and glomerular filtration rate. Different profiles based on eGFR and spot urine sodium concentration are associated with different risks of in-hospital heart failure worsening, inotrope use, and rehospitalization due to AHF. The profile of preserved eGFR and non-excreter of sodium is associated with the highest 1-year mortality and remains an independent prognosticator after adjustment for other prognosticators.

Natriuretic Equation to Predict Loop Diuretic Response in Patients With Heart Failure

Veena S. Rao et al.

Summary: The study validated the natriuretic response prediction equation (NRPE) for predicting sodium output and demonstrated its potential for guiding diuretic therapy in acute decompensated heart failure. Results showed that the NRPE provided accurate predictions and led to significant improvements in urine output, net fluid output, and weight loss when used to guide diuretic therapy, indicating its potential clinical utility. Further studies on diuresis guided by the NRPE are warranted to confirm its effectiveness.

Compensatory post-diuretic renal sodium reabsorption is not a dominant mechanism of diuretic resistance in acute heart failure

Zachary L. Cox et al.

Summary: In the study of patients with ADHF receiving loop diuretics, it was found that CPDSR was not a major driver of diuretic resistance. Instead, a greater diuretic-induced natriuresis predicted a larger post-diuretic spontaneous natriuresis.
