Hemodynamic Modeling, Medical Imaging, and Machine Learning and Their Applications to Cardiovascular Interventions

Mason Kadem et al.

Summary: Cardiovascular disease presents a global health crisis with significant financial consequences. Innovative approaches such as personalized hemodynamic modeling, machine learning, and modern imaging have been employed to enhance patient outcomes and decrease the economic burden. Hemodynamic modeling offers a non-invasive method to provide valuable metrics for clinicians, while medical imaging plays a crucial role in understanding and managing cardiac disease. By combining machine learning with modeling and cardiovascular imaging, it is possible to improve modeling speed, data accuracy, and early detection of cardiovascular anomalies, leading to the development of patient-specific diagnostic and predictive tools. This review provides an overview of translational hemodynamic modeling, medical imaging, and machine learning, highlighting their potential in supporting decision making during critical clinical milestones.

The Critical Role of Lumped Parameter Models in Patient-Specific Cardiovascular Simulations

Louis Garber et al.

Summary: Cardiovascular computational modelling, especially using lumped parameter modelling (LPM), is becoming increasingly important in early diagnosis, personalized treatment, and research of cardiovascular diseases. LPM's advantages of rapid computation time and simplicity make it a potential tool for developing patient specific cardiovascular digital twin frameworks in the future.

Reducing Morbidity and Mortality in Patients With Coarctation Requires Systematic Differentiation of Impacts of Mixed Valvular Disease on Coarctation Hemodynamics

Summary: The study showed that COA and mixed valvular diseases interacted to worsen abnormal blood flow dynamics and increase disease progression. Additionally, the presence of valvular diseases in patients led to increased workload, highlighting the importance of considering both COA severity and mixed valvular diseases in patient evaluation and treatment planning.

Edoxaban Versus Dual Antiplatelet Therapy for Leaflet Thrombosis and Cerebral Thromboempolism After TAVR: The ADADT-TAVR Randomized Clinical Trial

Duk-Woo Park et al.

Summary: This study compared the efficacy of the direct oral anticoagulant edoxaban with dual antiplatelet therapy in patients who had undergone transcatheter aortic valve replacement. The results showed a trend towards a lower incidence of leaflet thrombosis in the edoxaban group, but the difference was not statistically significant. There were no significant differences in the number and volume of new cerebral lesions or neurological and neurocognitive function between the two groups.

Clinical Impact of Hypoattenuating Leaflet Thickening After Transcatheter Aortic Valve Replacement

Santiago Garcia et al.

Summary: This study found that hypoattenuated leaflet thickening (HALT) was present in 12% of patients who underwent transcatheter aortic valve replacement (TAVR) and was independently associated with long-term mortality. The findings highlight the importance of systematic screening with functional cardiac computed tomography (CTA) in identifying HALT and its impact on clinical outcomes in TAVR patients.

Impact of mixed valvular disease on coarctation hemodynamics using patient-specific lumped parameter and Lattice Boltzmann modeling

Summary: The optimal course of intervention for patients with coexisting coarctation of the aorta (COA) and mixed valvular disease (MVD) is uncertain. This study developed a computational-mechanics framework to investigate the impact of COA and MVD on aortic fluid dynamics. The results suggest that the presence and severity of MVD should be considered in the evaluation of risks in patients with COA.

On Left Ventricle Stroke Work Efficiency in Children with Moderate Aortic Valve Regurgitation or Moderate Aortic Valve Stenosis

Summary: The optimal timing for management of pediatric patients with moderate aortic valve disease remains unknown. A new evaluative parameter of LV performance, LVSW efficiency, may offer a promising avenue for future stratification of mixed aortic valve disease for optimal timing of management and intervention. Patients with moderate aortic valve disease have significantly reduced LV performance and may require careful myocardial assessment for timely intervention.

Summary: This study found that frame deformation, asymmetric leaflet, and smaller neosinus volume are associated with the occurrence of HALT in patients undergoing TAVR. The presence of HALT is associated with an increased risk of mortality within 1 year.

The role of flow stasis in transcatheter aortic valve leaflet thrombosis

Phillip M. Trusty et al.

Summary: This study is the first patient-specific study that correlates flow stasis with thrombus volume in the neosinus post-transcatheter aortic valve replacement, covering multiple valve types and sizes. The results show a significant positive correlation between flow stasis and thrombus volume. Neosinus-specific factors lead to hemodynamic and thrombotic variability among individual patients. Measurement of neosinus flow stasis may guide strategies to improve outcomes in transcatheter aortic valve replacement.

Hemodynamics inside the neo- and native sinus after TAVR: Effects of implant depth and cardiac output on flow field and coronary flow

Desiree Pott et al.

Summary: This study investigated the effect of different prosthesis implant depths on flow field within the neo- and native sinuses and coronary perfusion following TAVR surgery. The results showed that a lower implant position and lower cardiac output led to better washout in the native sinus, while a higher implant position improved coronary flow. The study concluded that the choice of implant position impacts flow dynamics and coronary perfusion post-TAVR.

Subclinical Leaflet Thrombosis After Transcatheter Aortic Valve Replacement A Meta-Analysis

Matthias Bogyi et al.

Summary: This meta-analysis evaluated the clinical relevance of subclinical leaflet thrombosis (SLT) post transcatheter aortic valve replacement. The use of intra-annular valves was associated with a higher risk of SLT, while oral anticoagulation showed a significant risk reduction. Further studies are needed to assess the long-term effects of screening tests and antithrombotic therapy on valve functionality and clinical prognosis.
