Birth outcomes in women with body mass index of 40 kg/m2 or greater stratified by planned and actual mode of birth: a systematic review and meta-analysis

Rohan D'Souza et al.

Summary: This systematic review compared outcomes in pregnant women with a BMI >= 40 kg/m(2) based on planned and actual mode of birth. The results suggest that different modes of delivery may lead to different risks of postpartum complications.

Adipose tissue inflammation and metabolic dysfunction in obesity

Tatsuo Kawai et al.

Summary: Chronic low-grade inflammation of adipose tissue is linked to metabolic disease and organ complications, mainly caused by the secretion of inflammatory substances by adipocytes and infiltration of immune cells. Despite being low-grade, this inflammation negatively impacts remote organ function and is considered a causative factor of obesity-related complications.

Birth Outcomes of Women with Obesity Enrolled for Care at Freestanding Birth Centers in the United States

Cecilia M. Jevitt et al.

Summary: Primiparous women with obesity can safely and effectively receive midwifery care at freestanding birth centers with a low risk for cesarean birth. There were no significant differences in perinatal outcomes between women with obese BMIs and normal BMIs, with the majority of women with obese BMIs experiencing uncomplicated perinatal courses and vaginal births.

Adipose tissue and insulin resistance in obese

Bulbul Ahmed et al.

Summary: This review discusses the potential mechanisms behind obesity-associated insulin resistance, such as dysfunctional adipose tissue, excessive release of free fatty acids, and the impact of lipotoxicity on cellular dysfunction leading to systemic inflammation and disruption of the insulin signaling pathway. It highlights that hepatic insulin resistance is the primary event that triggers peripheral tissue insulin resistance.

An Empirically Derived Definition of Metabolically Healthy Obesity Based on Risk of Cardiovascular and Total Mortality

Anika Zembic et al.

Summary: The study derived a new definition of metabolic health (MH) using data from NHANES-III and UK Biobank cohorts, and found that metabolically healthy obesity is not associated with increased risk of cardiovascular disease (CVD) mortality and total mortality compared to metabolically healthy normal weight individuals. The new MH definition may help identify a subgroup of obese individuals without increased mortality risk and stratify risk in overweight or normal weight individuals.

Summary: This review suggests that supporting increased physical activity and appropriate weight gain during early pregnancy, as well as optimizing weight and health behaviors prior to and between pregnancies, may help reduce the risk of GDM. Further development and evaluation of interventions are needed to determine how to reach and support women to improve their health behaviors across community, health service, and policy levels.
