Comprehensive Management of Cardiovascular Risk Factors for Adults With Type 2 Diabetes: A Scientific Statement From the American Heart Association

Joshua J. Joseph et al.

Summary: This scientific statement highlights the importance of controlling cardiovascular risk factors in reducing cardiovascular events in patients with diabetes. It analyzes how newer antihyperglycemic agents, blood pressure control, and lipid-lowering therapies can improve glycemic control and prevent cardiovascular disease in diabetes. Additionally, it emphasizes the significance of lifestyle interventions, pharmacological therapy, and surgical interventions in managing obesity and metabolic syndrome, which are important precursors of diabetes and comorbid cardiovascular disease. Furthermore, it explores the critical role of social determinants of health and health equity in the continuum of care for diabetes and cardiovascular disease.

Guidelines for Cardiovascular Risk Reduction in Patients With Type 2 Diabetes JACC Guideline Comparison

Michelle D. Kelsey et al.

Summary: Cardiovascular disease is a major concern for individuals with type 2 diabetes mellitus, and aggressive risk factor management is crucial for these high-risk patients. Recommendations and guidelines from professional societies in cardiology, endocrinology, nephrology, and general medicine in the United States and Europe provide guidance on cardiovascular risk reduction for patients with type 2 diabetes mellitus. While there are some differences in risk stratification, treatment targets for low-density lipoprotein-cholesterol and blood pressure, as well as the use of specific medications, overall there is significant alignment across recommendations from different professional societies.

Pharmacotherapy of type 2 diabetes: An update and future directions

Antea DeMarsilis et al.

Summary: This review provides an updated summary of the pharmacotherapeutic approach to type 2 diabetes (T2D), including the mechanism of action, administration details, side effect profile, cost, and use in specific populations of different classes of agents. It also reviews the targets of novel therapeutic T2D agent development. Finally, it outlines a contemporary treatment approach starting with the identification of an individualized glycemic control goal, followed by the selection, initiation, and further intensification of a personalized therapeutic plan for T2D.

Type 2 diabetes

Ehtasham Ahmad et al.

Summary: Type 2 diabetes accounts for nearly 90% of global diabetes cases, with an increasing number of affected individuals, particularly among young people. Early detection and proactive management are crucial for preventing complications and reducing mortality. Technology and precision medicine offer potential improvements to treatment outcomes but face significant barriers.

Nirmatrelvir-remdesivir association for non-hospitalized adults with COVID-19, point of view

Francesco Ferrara et al.

Summary: The scientific community is actively searching for new antiviral drugs and therapies to combat SARS-CoV-2. Paxlovid, an oral antiviral, has recently been authorized and has shown good efficacy in reducing the risk of hospitalization in COVID-19 patients. Remdesivir is another authorized antiviral for treating SARS-CoV-2 infection. However, there is still limited data in the literature regarding the safety profile and risk of generating antiviral-resistant variants. This manuscript discusses the monotherapy use of paxlovid and remdesivir, as well as the scientific hypothesis of combining nirmatrelvir and remdesivir to increase treatment efficacy, reduce adverse reactions, and prevent antiviral drug resistance.

Cardiogenic Shock After Acute Myocardial Infarction A Review

Marc D. Samsky et al.

Summary: Cardiogenic shock affects 40,000 to 50,000 people in the US annually and is a leading cause of in-hospital mortality post acute myocardial infarction. Immediate revascularization of the infarct-related coronary artery is the primary treatment recommended, supported by clinical trials. Despite lack of definitive evidence, mechanical circulatory support devices are frequently utilized in managing cardiogenic shock.

Summary: Direct dispensing (DD) has many advantages over dispensing on commission (DPC), including greater therapeutic appropriateness and cost savings. However, a complex warehouse organization is a major challenge for DD. With the increase in chronic patients and advanced therapies, direct care requires a new structure and logistics system.
