Covered Stents in the Management of Aortic Coarctation and Right Ventricular Outflow Tract Obstruction

Nibras E. El Sherif et al.

Summary: This review examines the use of covered stents in the treatment of coarctation of the aorta and right ventricle to pulmonary artery conduit obstruction. The only commercially available covered stent approved for these conditions is the covered Cheatham-Platinum stent. Early outcomes have shown its safety and efficacy in preventing aortic wall injury or conduit disruption. However, a recent study reported a progressive risk of stent fracture over time and a risk of aortic wall injury despite the purported protection of the covered stent.

Endovascular Stent Repair of Aortic Coarctation in a Developing Country: A Single-Center Experience

Mouafak Homsi et al.

Summary: This study evaluated the safety, feasibility, and outcomes of endovascular stenting for Coarctation of the aorta (CoA) in a developing country with limited resources. The results showed that endovascular stenting is a feasible and safe treatment option for de-novo or recurrent CoA in children and adults in a developing country, with outcomes comparable to developed countries.

Sequential dilation strategy in stent therapy of the aortic coarctation: A single centre experience

Pinar Bambul Heck et al.

Summary: The study demonstrated that interventional aortic stenting with sequential dilation is effective in reducing systolic pressure gradient and controlling blood pressure in CoA patients. The procedure showed a low rate of re-interventions and complications, leading to a better control of hypertension with adjusted antihypertensive medication.

Vascular Ageing and Aerobic Exercise

Michaela Kozakova et al.

Summary: The impairment of vascular function with aging, particularly endothelial dysfunction and large elastic artery stiffening, is closely linked to oxidative stress and inflammation. Aerobic exercise has been shown to improve vascular function, reduce inflammation, and lower cardiovascular risk associated with aging.

Long-Term Outcomes of the Coarctation of the Aorta Stent Trials

Ralf J. Holzer et al.

Summary: Long-term follow-up data from the COAST and COAST II trials showed that stent-implantation for coarctation of the aorta can effectively maintain obstruction relief for up to 5 years, reducing the need for antihypertensive medication. However, there was an observed increase in stent fractures and reinterventions in the medium to long term.

Summary: This study described the safety and efficacy of covered stents in patients with CoA for immediate and long-term follow-up. Results showed a significant improvement in hemodynamics after stent implantation, but some patients needed antihypertensive medication during follow-up.
