A review on initiatives for the management of daily medical emergencies prior to the arrival of emergency medical services

Niki Matinrad et al.

Summary: Emergency services worldwide are facing challenges such as increasing cost pressure, staff shortage, and longer response times, especially during crisis times like the COVID-19 pandemic. To address these issues and reduce consequences of daily medical emergencies, initiatives using new types of human resources and equipment have been started in countries like Sweden, Germany, and the Netherlands.

Rapid response systems The association of fire or police first responder initiated interventions with out of hospital cardiac arrest survival

Rama A. Salhi et al.

Summary: This study evaluated the rates of CPR initiation and AED use by fire or police first responders in out-of-hospital cardiac arrest incidents. The results showed that interventions by these first responders were associated with higher odds of survival to hospital discharge and good neurologic outcome, indicating their potential effectiveness in improving OHCA survival.

Analyzing the heart rhythm during chest compressions: Performance and clinical value of a new AED algorithm

Corina de Graaf et al.

Summary: The study assessed the diagnostic accuracy and clinical benefits of a new AED algorithm, cprINSIGHT, which showed 96% sensitivity and 98% specificity during chest compressions. Compared to conventional AEDs, the intervention group had a significantly shorter pre-shock pause and a higher chest compression fraction.

Management of first responder programmes for out-of-hospital cardiac arrest during the COVID-19 pandemic in Europe

Linn Andelius et al.

Summary: In response to the COVID-19 pandemic, most regions in Europe adjusted the management of FR programmes. The majority either implemented restrictions or temporarily paused the programmes, with common measures including omission of rescue breaths for citizen responders and provision of personal protective equipment for professional FRs. Further studies are needed to explore the impact of these changes on bystander CPR and AED use, as well as patient outcomes.

European first responder systems and differences in return of spontaneous circulation and survival after out-of-hospital cardiac arrest: A study of registry cohorts

Iris Oving et al.

Summary: The presence of a dispatched First Responder (FR) system in European regions may be associated with higher rates of return of spontaneous circulation (ROSC) and survival for out-of-hospital cardiac arrest (OHCA) patients. Regions with FR systems showed significantly higher ROSC and survival rates compared to regions without, indicating the importance of having FR systems for improving outcomes of OHCA patients.
