Aspirin in essential thrombocythemia. For whom? What formulation? What regimen?

Summary: Essential thrombocythemia (ET) is a myeloproliferative neoplasm characterized by abnormal platelet production. Low-dose aspirin is recommended as the standard treatment for controlling vascular events in ET patients. However, some patients may display poor responsiveness to enteric coated aspirin. Twice daily aspirin administration has shown to overcome this problem and is considered safe with a favorable risk/benefit profile. The once daily regimen can still be used in patients with normalized platelet count to minimize potential gastrointestinal discomfort.

Summary: Aspirin is widely used for the prevention of recurrent ischemic events in patients with atherosclerotic disease. Current formulations of aspirin have limitations, such as high risk of gastrointestinal adverse events and erratic absorption. The novel formulation of phospholipid-aspirin complex (PL-ASA) addresses these needs by reducing acute gastrointestinal injury and providing predictable absorption for reliable platelet inhibition.

The impact of enteric coating of aspirin on aspirin responsiveness in patients with suspected or newly diagnosed ischemic stroke: prospective cohort study: results from the (ECASIS) study

Summary: This study aimed to compare the effects of enteric-coated aspirin and plain aspirin on patients with ischemic stroke. The results showed no significant difference in aspirin non-responsiveness between the two formulations. Further research is needed to explore the risk of aspirin non-responsiveness in diabetic patients.
