Summary: Most individuals with congenital heart disease now survive to adulthood and need to transfer from pediatric to adult-oriented health care systems. Developing transition skills and independence in managing one's own health care is crucial. This statement reviews evidence on transition and provides resources and suggestions for the development of congenital heart disease transition programs to improve patient knowledge and self-management skills.

Transfer and transition practices in 96 European adult congenital heart disease centres

Summary: The study surveyed existing transfer and transition programmes in European ACHD centres and found that nearly half of the centres offered a formal transition programme, while almost all centres had structured transfer from paediatric to ACHD care. However, most programs lacked an age and development-based approach despite some improvements since 2009.

Discontinuity of Cardiac Follow-Up in Young People With Congenital Heart Disease Transitioning to Adulthood: A Systematic Review and Meta-Analysis

Summary: The meta-analysis revealed a high proportion of discontinuity of care in young people with congenital heart disease, especially in studies from the United States and patients with simple heart defects. Transition programs were suggested to have a protective effect in reducing discontinuity of care.
