Hybrid APC in Combination With Resection for the Endoscopic Treatment of Neoplastic Barrett's Esophagus: A Prospective, Multicenter Study

Mate Knabe et al.

Summary: This study evaluated a potential alternative treatment for neoplastic Barrett's esophagus, called hybrid argon plasma ablation. The results showed that this technique achieved the expected eradication and recurrence rates of Barrett's intestinal metaplasia and neoplasia within 2 years. However, a randomized comparative trial is needed to provide final evidence compared to radiofrequency ablation.

Outcomes of Radiofrequency Ablation Compared to Liquid Nitrogen Spray Cryotherapy for the Eradication of Dysplasia in Barrett's Esophagus

Matthew Fasullo et al.

Summary: This study compared the efficacy of radiofrequency ablation (RFA) and liquid nitrogen spray cryotherapy (LNSC) for Barrett's esophagus (BE), finding that while LNSC required more treatment sessions, both methods achieved similar rates of complete eradication of dysplasia and neoplasia. Achieving complete eradication of intestinal metaplasia (CE-IM) was associated with lower recurrence rates of dysplasia.

Diagnosis and Management of Barrett's Esophagus: An Updated ACG Guideline

Nicholas J. Shaheen et al.

Summary: Barrett's esophagus is a common condition associated with chronic gastroesophageal reflux disease and is a precursor to esophageal adenocarcinoma. The revised guidelines propose recommendations for the definition, diagnosis, screening, surveillance, and treatment of Barrett's esophagus, including important changes such as expanding acceptable screening methods and volume criteria for treatment centers.

Comparative outcomes of radiofrequency ablation and cryoballoon ablation in dysplastic Barrett's esophagus: a propensity score-matched cohort study

Siddharth Agarwal et al.

Summary: This study compared the effectiveness and safety of radiofrequency ablation (RFA) and cryoballoon ablation (CBA) in the treatment of dysplastic Barrett's esophagus (BE). The histologic outcomes of endoscopic eradication therapy (EET) using RFA and CBA were found to be comparable. A randomized trial is needed to definitively compare outcomes between these two modalities.

Efficacy and Safety of Radiofrequency Ablation vs. Endoscopic Surveillance for Barrett's Esophagus With Low-Grade Dysplasia: Meta-Analysis of Randomized Controlled Trials

Yizi Wang et al.

Summary: Radiofrequency ablation (RFA) reduces the risk of progression from low-grade dysplasia (BE-LGD) to high-grade dysplasia (BE-HGD). RFA treatment is associated with a higher rate of complete eradication of dysplasia and intestinal metaplasia, but also a slightly higher rate of adverse events.

Argon plasma coagulation for Barrett's esophagus with low-grade dysplasia: a randomized trial with long-term follow-up on the impact of power setting and proton pump inhibitor dose

Ewa Wronska et al.

Summary: This study evaluated the impact of power setting and proton pump inhibitor dose on the efficacy and safety of argon plasma coagulation for treating Barrett's esophagus with low-grade dysplasia. The results showed that power setting and PPI dose did not significantly affect the ablation of Barrett's esophagus, with over 90% of patients maintaining complete ablation in the long term without evidence of neoplasia progression.

Summary: This study found through a randomized trial that radiofrequency ablation therapy can moderately reduce the prevalence of low-grade dysplasia (LGD) in Barrett's esophagus over 3 years and decrease the risk of progression. The complication rate was highest after the first radiofrequency ablation treatment.

Summary: The US pilot study on hybrid APC for Barrett's esophagus ablation showed promising efficacy, tolerance, and safety. The results suggest that hybrid APC may be a cost-effective alternative for BE treatment compared to other modalities. Further research in a large prospective multicenter trial is warranted.
