Biomarkers as Prognostic Predictors and Therapeutic Guide in Critically Ill Patients: Clinical Evidence

Summary: A biomarker is a measurable molecule in a biological sample that can objectively indicate whether a process is normal or pathological. Understanding the important biomarkers and their characteristics is crucial for precision medicine in intensive and perioperative care. Biomarkers play various roles in diagnosing, assessing disease severity, stratifying risk, predicting clinical outcomes, guiding treatments, and evaluating response. This review discusses the characteristics of biomarkers and how to ensure their usefulness, while also introducing several biomarkers that are considered valuable in clinical practice. It also proposes an approach to perioperative evaluation based on biomarkers for high-risk and critically ill patients in the ICU.

Preoperative Neutrophil and Lymphocyte Ratio as a Predictor of Mortality and Morbidity After Cardiac Surgery

Summary: This study investigated the association between preoperative neutrophil to leucocyte ratio (NLR) and postoperative outcomes in patients undergoing cardiac surgery. A high NLR of 3.23 or greater was found to be associated with increased mortality and higher risk of postoperative complications such as atrial fibrillation, stroke, new neurological deficits, and increased mortality. The interpretation and utilization of readily available hematological markers can provide valuable risk stratification data for surgeons when considering postoperative risks in cardiac surgery.

Phase Angle and Handgrip Strength as Complements to Body Composition Analysis for Refining Prognostic Accuracy in Cardiac Surgical Patients

Summary: This study found that cardiac surgery patients experience significant impairment in nutritional status, body composition parameters, and muscle strength postoperatively, leading to prolonged hospital stay. Postoperative phase angle, fat-free mass index, and extracellular water expansion were identified as strong predictors of extended hospital stay, with phase angle and extracellular water also acting as predictors of in-hospital mortality. Postoperative phase angle, extracellular water, and total body water were found to have superior or at least equivalent predictive power for morbidity and mortality compared to EuroSCORE II.
