Comparison of phrenic nerve injury and its effect on the extrapulmonary vein structures with cryoballoon and hot-balloon ablation systems: considering the lesion created on the superior vena cava as a surrogate marker

Yuichi Hori et al.

Summary: Preventing phrenic nerve injury is essential in balloon-based ablation. Cryoballoon ablation (CBA) creates more substantial ablation effects at the SVC-RA junction compared to hot-balloon ablation (HBA). This may be influenced by the different characteristics of the two balloon-based ablation systems and their balloon positions.

Role of empirical isolation of the superior vena cava in patients with recurrence of atrial fibrillation after pulmonary vein isolation-a multi-center analysis

Sven Knecht et al.

Summary: This study investigated the feasibility of an empirical strategy of additional SVC isolation (SVCI) in patients with recurrent atrial fibrillation (AF) after index pulmonary vein isolation (PVI). The results showed that the empirical strategy of additional SVCI did not improve outcomes compared to the PVI-only approach in patients with recurrent AF.

Empirical superior vena cava isolation in patients undergoing repeat catheter ablation procedure after recurrence of atrial fibrillation

Gelu Simu et al.

Summary: This study aimed to investigate the effect of empirical superior vena cava (SVC) isolation in redo atrial fibrillation (AF) ablation procedures. The results showed that adding SVC isolation to repeat AF ablation procedures did not affect AF recurrence rates, but this strategy could be safe and useful for patients in whom the SVC is identified as a trigger of AF.

Summary: This study investigated the incidence of non-responders to pulmonary vein isolation (PVI) in patients undergoing a repeat procedure and identified predictors for recurrence. The study found that non-paroxysmal AF and diabetes mellitus were associated with recurrence, and empirical superior vena cava (SVC) isolation may improve outcomes in non-responders to PVI.

Predictors of non-pulmonary vein foci in paroxysmal atrial fibrillation

Keita Watanabe et al.

Summary: The study identified female gender, lower BMI, absence of hypertension, and higher E/e' ratio as significant predictors of non-PV foci in patients with paroxysmal AF. Having all four factors was associated with 96.8% specificity in detecting non-PV foci. These factors could be useful in selecting a device for performing pulmonary vein isolation.

Summary: Successful elimination of non-pulmonary vein triggers can reduce the recurrence rate of atrial tachyarrhythmia after ablation for persistent atrial fibrillation, while higher recurrence rate was observed in patients with persistent atrial fibrillation if non-PV AF triggers or even non-PV PACs remain.

Distribution of triggers foci and outcomes of catheter ablation in atrial fibrillation patients in different age groups

Summary: Young AF patients have a higher incidence of only non-PV foci, mostly located in the SVC, compared to middle-aged and old patients. While the LA mean voltage was higher and the very late recurrence rate after AF ablation was lower in the young group, final AF recurrence rates and complication rates were similar among all age groups after a mean follow-up of 5.6 years.
