A Prospective Cohort Study of Novel Markers of Hepatitis B Virus Replication in Human Immunodeficiency Virus Coinfection

Raymond T. Chung et al.

Summary: This study evaluated the role of HBV RNA and HBcrAg in characterizing HBV-HIV coinfection and found that the use of novel HBV markers can continue to improve the suppression of HBV transcription and translation in suppressive dually active antiviral therapy. However, for HBeAg patients, the benefit of combination antiviral therapy is limited.

Accuracy of FIB-4 and APRI scores compared to transient elastography for liver fibrosis in patients with HIV and HBV co-infection

Rongrong Yang et al.

Summary: In China, where HIV/HBV co-infection is prevalent, liver biopsy and transient elastography (TE) are rarely available due to economic shortages and concerns about occupational exposure to HIV. This study compared the accuracy of FIB-4 and APRI for predicting liver fibrosis with TE results in 460 HIV/HBV co-infected patients. The results showed a strong correlation between FIB-4 and APRI scores and TE measurements of liver stiffness, indicating that these indexes can be accurate, simple, and inexpensive methods for assessing liver fibrosis in patients with HIV/HBV co-infection in resource-limited settings.

Hepatitis B Surface Antigen Levels Can Be Used to Rule Out Cirrhosis in Hepatitis B e Antigen-Positive Chronic Hepatitis B: Results From the SONIC-B Study

Milan J. Sonneveld et al.

Summary: This study investigated the relationship between HBsAg levels and hepatic fibrosis and cirrhosis in patients with chronic hepatitis B. The results showed that lower HBsAg levels were associated with higher rates of fibrosis and cirrhosis in HBeAg-positive patients, but not in HBeAg-negative patients. Additionally, genotype-specific HBsAg cutoffs could be used to rule out the presence of cirrhosis. These findings are important for the diagnosis and treatment of chronic hepatitis B patients.

Hepatitis B surface antigen and hepatitis B RNA changes in HIV/hepatitis B virus co-infected participants receiving hepatitis B virus-active antiretroviral therapy

Claudia Hawkins et al.

Summary: This study aims to identify serum biomarkers that assess the HBV cccDNA reservoir and predict functional cure in HIV/HBV co-infection. The results support the use of TDF-based DUAL-HBV active ART as the initial treatment option for HIV/HBV co-infection and suggest that HBV RNA could be a useful marker for evaluating treatment response in these patients.

HBV pgRNA profiles in Chinese HIV/HBV coinfected patients under pre- and posttreatment: a multicentre observational cohort study

Ling Xu et al.

Summary: This study aimed to evaluate the distribution of hepatitis B virus (HBV) pregenomic (pgRNA) levels in HIV/HBV coinfected patients and explore the changes that occur after the initiation of combined antiretroviral therapy (cART). The results showed that HBeAg-negative individuals had significantly lower HBV pgRNA levels than HBeAg-positive individuals before treatment, and HBV pgRNA was associated with HBsAg and HBV DNA. HBV DNA and HBeAg status were independently associated with undetectable HBV pgRNA levels before cART. After cART, some patients had undetectable HBV pgRNA levels, but there was no HBsAg loss.

PAGE-B and REACH-B Predicts the Risk of Developing Hepatocellular Carcinoma in Chronic Hepatitis B Patients from Northeast, Brazil

Alessandra Porto de Macedo Costa et al.

Summary: This study evaluates the accuracy of PAGE-B and REACH-B scores in predicting the risk of developing hepatocellular carcinoma (HCC) in patients with chronic hepatitis B. The study found that thrombocytopenia, high viral load, male gender, and age are associated with the occurrence of HCC. PAGE-B and REACH-B scores are able to predict the risk of developing HCC, which can help reduce screening costs.

Increased HBV Coinfection and Decreased IFN-γ-Producing HBV-Specific CD8+T Cell Numbers During HIV Disease Progression

Zhiqiang Zhu et al.

Summary: The coinfection rate of HBV in HIV patients increases as HIV disease progresses, possibly due to the reduced numbers of HBV-specific IFN-gamma-producing CD8+ T cells. Close monitoring of HBV serum markers from the early stage of HIV infection is necessary.

Circulating IL-1β, IL-17, and IP-10 as Potential Predictors of Hepatitis B Virus Infection Prognosis

Zhenzhen Su et al.

Summary: This study explored the effects of cytokines and chemokines on hepatitis B virus infection. The results showed that interleukin-17 may help in HBV clearance and protect against HBV-related hepatoma development, while IL-1 beta negatively affected HBV clearance. In patients with chronic hepatitis B, interferon-gamma-inducible protein-10 levels were significantly associated with abnormal liver function. Inflammatory cytokines and chemokines may have a dual role in HBV clearance and progression.

Summary: In this study, it was found that IL-17 may facilitate HBV spontaneous clearance and protect against HBV-related hepatoma development, while IL-1 beta negatively affects HBV clearance. In patients with chronic hepatitis B, IP-10 levels significantly increase in those with abnormal liver function, showing positive correlations with liver enzyme levels. Further exploration of the roles of IL-17, IL-1 beta, and IP-10 in chronic HBV infection is warranted.

Combination antiretroviral therapy is associated with reduction in liver fibrosis scores in patients with HIV and HBV co-infection

Rongrong Yang et al.

Summary: This retrospective observational study found that cART was a protective factor against liver fibrosis in individuals with HIV/HBV co-infection. Among patients with normal levels of ALT, AST and PLT, those with higher FIB-4 scores before cART tended to see greater reductions in FIB-4 score after cART initiation. Additionally, the benefits of cART in reducing liver fibrosis were sustained over a decade in these patients.
