Mechanisms underlying capsulotomy for refractory obsessive-compulsive disorder: neural correlates of negative affect processing overlap with deep brain stimulation targets

Summary: Ablative procedures like anterior capsulotomy can effectively treat refractory OCD. A study found that white matter tracts in the ventral internal capsule connecting the rostral cingulate, ventrolateral prefrontal cortex, and thalamus are the optimal target for clinical efficacy in OCD deep brain stimulation. Using fMRI and neuropsychological tests, the study investigated the prefrontal regions and cognitive processes involved in capsulotomy effects. Results showed improved OCD symptoms and quality of life in post-capsulotomy patients, with changes in brain activity and connectivity in regions associated with OCD. These findings contribute to the optimization of neuromodulation approaches for OCD.

Differential assessment of frontally-mediated behaviors between self- and informant-report in patients with obsessive-compulsive disorder following gamma ventral capsulotomy

Summary: This study investigated the changes in frontally-mediated behaviors following gamma ventral capsulotomy (GVC) surgery in patients with severe and refractory OCD. The results showed a significant improvement in self-ratings of frontal systems behavior after the surgery, while family ratings did not change significantly. The improvement in frontally-mediated behaviors was also correlated with improvement in OCD symptoms post-surgery.
