Spanish COPD Guidelines (GesEPOC) 2021 Update Diagnosis and Treatment of COPD Exacerbation Syndrome

Juan Jose Soler-Cataluna et al.

Summary: This article provides detailed recommendations on the diagnosis and treatment of COPD exacerbation syndrome (CES). The guidelines propose a syndromic approach, a new severity classification, and the recognition of different treatable traits (TT), aiming to personalize medicine. The diagnostic process includes four stages, and the treatment varies depending on severity and specific traits.

Effectiveness and Safety of Inhaled Antibiotics in Patients With Chronic Obstructive Pulmonary Disease. A Multicentre Observational Study

David De la Rosa Carrillo et al.

Summary: Inhaled antibiotics appear to be an effective and safe treatment for chronic obstructive pulmonary disease (COPD) patients, especially those with multiple exacerbations and/or chronic bronchial infection. This treatment can significantly reduce exacerbation frequency, hospital admissions, and hospitalization days, as well as decrease sputum production and eradicate potentially pathogenic microorganisms.

The Impact of Chronic Bronchial Infection in COPD: A Proposal for Management

Miguel Angel Martinez-Garcia et al.

Summary: Up to 50% of stable state chronic obstructive pulmonary disease (COPD) patients may carry potentially pathogenic microorganisms (PPMs) in their airways, which are associated with increased symptoms, exacerbations, decline in lung function, and impaired quality of life. Currently, there is a lack of evidence-based recommendations for evaluating and treating the presence of PPMs in COPD, but defining chronic bronchial infection (CBI) as the isolation of the same PPM in three sputum samples separated by at least one month and recommending antibiotic treatment based on specific criteria may help manage these challenging patients.

Chronic Bronchial Infection Is Associated with More Rapid Lung Function Decline in Chronic Obstructive Pulmonary Disease

Miguel Angel Martinez-Garcia et al.

Summary: The presence of chronic bronchial infection (CBI) and isolation of Pseudomonas aeruginosa (PA) are independently associated with forced expiratory volume in 1 second (FEV1) decline in patients with COPD.

Bronchial Infection and Temporal Evolution of Bronchiectasis in Patients With Chronic Obstructive Pulmonary Disease

Miguel Angel Martinez-Garcia et al.

Summary: This study aimed to assess the temporal evolution of bronchiectasis in patients with COPD. Results indicated that the presence of chronic purulent sputum production, number of pathogenic microorganism isolations in sputum, and number of hospitalizations due to exacerbations of COPD were independent risk factors for the progression or development of BE.

Risk Factors and Relation with Mortality of a New Acquisition and Persistence of Pseudomonas aeruginosa in COPD Patients

Miguel Angel Martinez-Garcia et al.

Summary: The isolation of Pseudomonas aeruginosa (PA) in patients with chronic obstructive pulmonary disease (COPD) is associated with increased mortality. Risk factors for first PA isolation and PA persistence are different in clinically stable COPD patients, with the latter associated with increased all-cause mortality.

Research priorities to address the global burden of chronic obstructive pulmonary disease (COPD) in the next decade

Davies Adeloye et al.

Summary: The global prevalence of COPD has been increasing, emphasizing the need for defining research priorities in respiratory medicine. Through the CHNRI methodology, new effective strategies for smoking cessation and feasible pulmonary rehabilitation delivery were identified as key research priorities. Additional priorities include improved screening and accurate diagnosis methods for COPD in low-resource settings.

Chronic bronchial infection and incident cardiovascular events in chronic obstructive pulmonary disease patients: A long-term observational study

Miguel Angel Martinez-Garcia et al.

Summary: This study investigated the relationship between chronic bronchial infection (CBI), systemic inflammation, and incident cardiovascular (CV) events in COPD patients. It found that CBI, especially by Pseudomonas aeruginosa (PA), is associated with sustained inflammation and a higher risk of CV events, particularly coronary events. Further research is needed to explore the potential benefits of treating CBI in reducing systemic inflammation and CV events in COPD.
