Summary: The incidence of hepatocellular carcinoma (HCC) is increasing in the elderly population in the United States, leading to a rise in transplant surgeries for this demographic. However, advanced age has been shown to be an independent predictor of overall survival posttransplantation, with every 5-year increase in age older than 50 years resulting in an 8.3% increase in mortality rate. Older patients also experience higher rates of non-HCC-related mortality, highlighting the importance of closer surveillance for de novo cancers in this population.
