Clinical and Dermoscopic Characteristics of Cutaneous Sarcomas: A Literature Review

Cutaneous Mesenchymal Sarcomas

Summary: Cutaneous mesenchymal sarcomas are rare diseases with limited guidelines. A summary of management is provided to assist clinicians. Surgical excision may lead to higher rates of local recurrence compared to common forms of nonmelanoma skin cancer. Patients with certain types of sarcomas are at higher risk of metastatic disease. Collecting data in large databases might help standardize the management of these rare carcinomas.

Atypical fibroxanthoma and pleomorphic dermal sarcoma - gene expression analysis compared with undifferentiated cutaneous squamous cell carcinoma

Summary: This study investigates the histogenetic origin of atypical fibroxanthoma (AFX) and pleomorphic dermal sarcoma (PDS). Through clinical, histomorphological, and immunohistochemical evaluations, similarities between AFX/PDS and undifferentiated cutaneous squamous cell carcinoma (cSCC) were discovered. RNA transcriptome analysis showed that AFX/PDS and differentiated cSCC formed their own clusters, while undifferentiated cSCCs fell in between. Differential gene expression analysis identified undifferentiated cSCC cases that were more likely to be AFX/PDS based on their expression profile. These findings provide evidence for molecular similarities and a common histogenetic origin between AFX/PDS and undifferentiated cSCC.

Surgical management of dermatofibrosarcoma protuberans

Summary: Dermatofibrosarcoma protuberans (DFSP) is a rare, locally aggressive skin malignancy. The primary treatment is complete resection, but the optimal method is debated. Mohs micrographic surgery is now recommended as the preferred approach according to National Comprehensive Cancer Network guidelines, replacing wide local excision. Medical therapy with imatinib can be used in advanced or unresectable cases. This review focuses on the current management of DFSP, with an emphasis on optimal surgical approach.

Kaposi's Sarcoma-Associated Herpesvirus Immediate Early Proteins Trigger FOXQ1 Expression in Oral Epithelial Cells, Engaging in a Novel Lytic Cycle-Sustaining Positive Feedback Loop

Summary: The oral cavity is an important site for virus entry and transmission. KSHV, an oncogenic gammaherpesvirus, can replicate in oral epithelial cells and promote viral transmission via saliva. FOXQ1, a host transcription factor, is induced by KSHV infection and plays a role in sustaining the viral lytic cycle in oral epithelial cells. Immediate early proteins ORF45 and RTA are involved in inducing FOXQ1 expression, and ORF45 uses a shared mechanism to induce both host and viral genes for lytic infection.

Atypical Fibroxanthoma: Outcomes from a Large Single Institution Series

Summary: This retrospective study provides clinical, pathologic, and treatment characteristics, as well as oncologic outcomes of AFX patients. The majority of AFX occurred in elderly, immunocompetent, Caucasian men, with most tumors located on the head and neck. Resection margins of 1 cm achieved excellent local control, with a recurrence rate close to 99% and 100% disease-specific survival.

Utility of Multimodal Treatment Protocols in the Management of Scalp Cutaneous Angiosarcoma

Summary: This study retrospectively reviewed cases of scalp cutaneous angiosarcoma diagnosed between 2005 and 2021. The analysis focused on resectability, defect reconstruction, and survival outcomes. The results showed that multimodal therapy combined with surgery had significantly better overall survival compared to surgery alone. Defect coverage was successfully achieved using various flap techniques. Further research is needed to explore advanced treatment modalities for this aggressive tumor.

A description of Kaposi sarcoma risk factors and outcomes in HIV-positive and HIV-negative patients at a tertiary care medical center from 2005 to 2020

Summary: Kaposi sarcoma (KS) is a vascular malignancy caused by human herpesvirus-8 (HHV-8). This study examined KS patients in Ohio, USA, and identified a fifth subtype called Unclassified-KS-Type. HIV-positive and HIV-negative patients were categorized, and the Unclassified-KS-Type patients were found to match the characteristics of the emerging non-epidemic subtype. This report provides updated information on KS risk factors and outcomes for improved detection and treatment in dermatology.

The 2020 WHO Classification of Soft Tissue Tumours: news and perspectives

The recent 2020 WHO classification of Soft Tissue Tumours and Bone, open to expert clinicians, has stimulated a debate on the importance of pathologic diagnosis and precision oncology. Pathologic diagnosis, integrating morphologic, immunohistochemical and molecular characteristics, plays a crucial role in clinical decision making and the WHO classification serves as a key instrument in promoting multidisciplinary efforts for more effective treatments.
