ILAE definition of the Idiopathic Generalized Epilepsy Syndromes: Position statement by the ILAE Task Force on Nosology and Definitions

Edouard Hirsch et al.

Summary: This paper aims to define the four syndromes comprising the idiopathic generalized epilepsies (IGEs) and provides updated diagnostic criteria. For patients who do not meet the criteria for these syndromes but have generalized seizure types, a classification is also provided. Recognizing these syndromes as a special grouping helps determine prognosis and treatment implications.

Centromedian Nucleus of the Thalamus Deep Brain Stimulation for Genetic Generalized Epilepsy: A Case Report and Review of Literature

Shruti Agashe et al.

Summary: There is a lack of treatment options for cognitively normal individuals with drug resistant genetic generalized epilepsy (GGE). Centromedian nucleus of the thalamus (CM) deep brain stimulation (DBS) may be an effective treatment for this population. However, further prospective studies are needed to validate its efficacy.

Drug resistance in idiopathic generalized epilepsies: Evidence and concepts

Joanna Gesche et al.

Summary: This review summarizes recent data and hypotheses on the mechanisms of drug-resistant idiopathic generalized epilepsy (IGE). The exact definition and cause of drug resistance in IGE remain unclear, but evidence suggests four different mechanisms that warrant further investigation.

The RNS System: brain-responsive neurostimulation for the treatment of epilepsy

Beata Jarosiewicz et al.

Summary: This review discusses the NeuroPace RNS System, the only brain-responsive neurostimulation device, and the results of multi-year clinical trials. Brain-responsive neurostimulation has the potential to be used for treating episodic neurological disorders with a neurophysiological biomarker of severity, and combining advanced machine learning with chronic ambulatory brain data collected by the RNS System could lead to personalized treatment for each patient.

Experience and consensus on stimulation of the anterior nucleus of thalamus for epilepsy

Alfonso Fasano et al.

Summary: Deep brain stimulation of the anterior nuclei of thalamus (ANT-DBS) is effective for reducing seizures in patients with temporal or frontal epilepsy refractory to at least two medications. Motivations for therapy renewal upon battery depletion include reduced seizures, improved quality of life, and reduction of severe seizures. Stimulation parameters are mainly guided by magnetic resonance imaging and typically follow parameters similar to the SANTE study, with initial stimulation amplitudes in the 2-3 V or mA range.
