Charlson Comorbidity Index: A Critical Review of Clinimetric Properties

Mary E. Charlson et al.

Summary: The Charlson Comorbidity Index (CCI) is a reliable assessment tool that predicts long-term mortality. It demonstrates excellent inter-rater reliability and concurrent validity, and is clinically sensitive and incrementally valid in various medical conditions.

Comparison of rapid molecular testing methods for detecting respiratory viruses in emergency care: a prospective study

Dagfinn Lunde Markussen et al.

Summary: This study investigated the efficacy of three rapid molecular methods for identifying patients with viral respiratory tract infections. The FilmArray(R) Pneumonia Panel plus (FAP plus) was found to significantly reduce turnaround time and accurately identify patients with potential contagious viral RTIs, suggesting its feasibility and importance in preventing nosocomial transmission of infections.

Factors Affecting Antibiotic Prescription among Hospital Physicians in a Low-Antimicrobial-Resistance Country: A Qualitative Study

Ingrid Christensen et al.

Summary: This study interviewed physicians in a Norwegian hospital to identify factors affecting their antibiotic choices. The main factors found included high work pressure, insufficient staff resources, uncertainties in clinical decisions, treatment expectations from patients and next of kin, benevolence towards patients, suboptimal microbiological testing, and limited time for infectious disease specialists to offer advisory services. To optimize antimicrobial stewardship programs, efforts should address time-pressure issues, improve microbiology testing, and enhance consultations with infectious disease specialists. Additionally, ethical debates about antibiotic questions among prescribing physicians are needed.

Antibiotic Prescriptions in Hospitalized Patients with an Exacerbation COPD and a Proven Influenza or RS Virus Infection

Sigrid van Brummelen et al.

Summary: The prescription of antibiotics in hospitalized COPD patients with a proven viral infection is common practice, despite guidelines providing limited guidance. This retrospective cohort study found that antibiotic treatment was correlated with X-ray infiltrates and plasma CRP levels, but did not significantly impact outcomes such as hospitalization days and mortality. The study suggests that a significant reduction in antibiotic use is feasible and future guidelines should include recommendations for antibiotic stewardship.

Clinical Outcome of Viral Respiratory Tract Infections in Hospitalized Adults in Norway: High Degree of Inflammation and Need of Emergency Care for Cases With Respiratory Syncytial Virus

Sara Debes et al.

Summary: The clinical features and outcomes of viral respiratory tract infections (RTIs) in adults, particularly respiratory syncytial virus (RSV) infections, have not been thoroughly studied. This study found that patients with RSV infections had higher levels of inflammation markers and disease severity compared to patients with other common respiratory viruses, but there were no differences in the need for intensive care unit (ICU) admission or mortality.

An Overview of Antimicrobial Stewardship Optimization: The Use of Antibiotics in Humans and Animals to Prevent Resistance

Md. Mominur Rahman et al.

Summary: Antimicrobial resistance is a major global concern that leads to the failure of clinical antimicrobial therapy and untreatable infections. Inappropriate use of antimicrobials has contributed to the development of resistance, rendering life-saving drugs ineffective. The causes of antimicrobial resistance are complex, including inadequate healthcare services, suboptimal use of antimicrobials, and environmental pollution.

Impact of rapid molecular testing on diagnosis, treatment and management of community-acquired pneumonia in Norway: a pragmatic randomised controlled trial (CAPNOR)

Sondre Serigstad et al.

Summary: This study aims to investigate whether the use of rapid molecular testing in hospitalized patients with suspected CAP can improve both the proportion of patients receiving pathogen-directed treatment and the time to treatment, thereby reducing unnecessary antibiotic use and hospital stay. The pragmatic design and broad inclusion criteria of this study may make the results applicable to other similar healthcare centers.

The influence of chest X-ray results on antibiotic prescription for childhood pneumonia in the emergency department

Josephine S. van de Maat et al.

Summary: This study found that in the emergency department, even in children suspected of lower respiratory tract infections, chest X-ray (CXR) examinations are still common. The performance of CXR was independently associated with more antibiotic prescriptions, regardless of its results. This highlights the limited influence of CXR results on antibiotic prescription and raises questions about the role of CXR in treatment decisions for suspected lower respiratory tract infections.

Etiology of viral respiratory tract infections in hospitalized adults, and evidence of the high frequency of prehospitalization antibiotic treatment in Norway

Sara Debes et al.

Summary: The study found high rates of respiratory viral infections in adult hospitalized patients, with influenza virus A/B, RSV A/B, and hMPV being the most common pathogens, and a significant proportion of patients receiving antibiotic treatment prior to hospital admission.
