Tyrosine kinase inhibitors-associated pyoderma gangrenosum, a systematic review of published case reports

Niloofar Khoshnam-Rad et al.

Summary: Pyoderma gangrenosum is a rare ulcerative dermatosis that can be caused by some drugs, including small molecule tyrosine kinase inhibitors (TKIs). This study evaluated the reported evidence of pyoderma gangrenosum associated with TKI use. Through a systematic electronic literature search, fifteen case reports associated with different TKIs were identified. The average Naranjo score of these cases was 6.6, indicating a probable adverse drug reaction. Detailed medical history can help prompt the diagnosis of drug-induced pyoderma gangrenosum. Clinicians should be aware of TKI-associated pyoderma gangrenosum in oncologic patients undergoing kinase inhibitor therapy.

Pyoderma gangrenosum study pilot registry: The first step to a better understanding

Victoria E. Orfaly et al.

Summary: The objective of this study was to develop a physician-driven patient registry for pyoderma gangrenosum (PG) to collect patient demographics, medical history, treatments, and outcomes. The pilot study found that the use of systemic immunomodulators was associated with healing of PG patients. Standardization of wound care regimens and data collection is crucial for evaluating treatment outcomes in PG.

Comparison of Three Diagnostic Frameworks for Pyoderma Gangrenosum

Carter Haag et al.

Summary: This study evaluated and compared the concordance of three different PG diagnostic frameworks within a cohort of 47 patients, finding that the PARACELSUS score identified the highest proportion of PG patients at 89%. The study highlights the need for future research to refine these diagnostic frameworks and identify targeted testing methods to reduce misdiagnosis rates and patient misclassification in clinical trials.

The surgical approach to Pyoderma gangrenosum: A retrospective monocenter study

Alperen S. Bingoel et al.

Summary: Pyoderma gangrenosum is a rare and frequently misdiagnosed disease that can result in severe tissue loss. Surgical treatments, such as debridements and skin grafts, are necessary for reconstruction once the wound progression is controlled. Drug-based therapy can stop the progress of the disease, but reconstructive-surgical treatments are an important component of therapy for selected patients.

The role of negative pressure wound therapy (NPWT) on the treatment of pyoderma gangrenosum: A systematic review and personal experience

Ivan R. Almeida et al.

Summary: Pyoderma gangrenosum (PG) is a rare and painful skin disease, and the use of negative pressure wound therapy (NPWT) in its treatment remains controversial due to the risk of pathergy. However, a systematic review showed that NPWT, when combined with immunosuppressive therapy, can improve wound healing in the majority of cases. Therefore, NPWT could be considered a safe option for treating PG wounds, especially when combined with systemic immunosuppression and skin grafting.

Summary: The evidence synthesis on anxiety and depression prevalence in adults with pyoderma gangrenosum (PG) showed that depression is a common psychological comorbidity in this patient population, with estimated rates ranging from 10% to 23%. However, none of the included studies measured rates of anxiety, indicating a need for further research in this area.

Summary: Pyoderma gangrenosum is a rare ulcerative neutrophilic dermatosis with diverse clinical presentations and challenging treatment. Accompanying comorbidities, including diabetes, were commonly found in patients. Tumor necrosis factor α inhibitors showed the shortest time to remission as a third- or fourth-line therapy, while the use of prednisone was associated with a higher mortality rate.
