Characteristics of and Outcomes After Dialysis-Treated Acute Kidney Injury, 2009-2018: A Taiwanese Multicenter Study

Summary: This study investigated the dialysis-treated acute kidney injury (AKI) in 9,535 adult ICU patients from Chang Gung Memorial Hospital system in Taiwan from 2009 to 2018. The study found a slight decrease in the severity of acute illness at dialysis initiation and a reduction in ICU mortality over time. However, there was an increase in dialysis treatment at discharge among survivors.

Acute Kidney Disease After Acute Decompensated Heart Failure

Summary: This study investigated the incidence and prognosis of acute kidney disease (AKD) after acute decompensated heart failure (ADHF). The results showed that AKD after ADHF was associated with adverse outcomes. A predictive model was developed to identify patients at risk for AKD development, especially those without a previous acute kidney injury (AKI) episode.

Nomenclature and diagnostic criteria for acute kidney injury-2020 consensus of the Taiwan AKI-task force

Summary: The Taiwan AKI-TASK Force conducted a comprehensive review on the definitions and diagnostic criteria of acute kidney injury, recommending the use of novel biomarkers, imaging examinations, renal biopsy, and body fluid assessment in diagnosis. While current classification systems using serum creatinine and urine output for AKI diagnosis may not be perfect, there is not enough evidence to change the current criteria in clinical practice at this time. Future research should focus on clarifying the roles of these tools in clinical practice for AKI.

Comparative accuracy of biomarkers for the prediction of hospital-acquired acute kidney injury: a systematic review and meta-analysis

Summary: This study compared the predictive performance of different biomarkers for acute kidney injury (AKI). The biomarkers containing NGAL showed the best predictive accuracy for the occurrence of AKI, especially in medically treated patients. However, the predictive performance of urinary NGAL was limited in surgical patients, and urinary NGAL/creatinine seemed to be the most accurate biomarkers in these patients. All of the biomarkers had similar predictive performance in critically ill patients.

Predialysis serum lactate levels could predict dialysis withdrawal in Type 1 cardiorenal syndrome patients

Summary: This study analyzed the association between lactate level and prognosis in patients with Type I CRS receiving RRT, and found that a low lactate level was associated with an increased likelihood of dialysis withdrawal and a lower mortality rate. Serum lactate level can be an important factor for clinical decision-making in treating patients with Type I CRS.

Summary: Risk stratification of patients with sepsis-associated acute kidney injury, using AKD stages, is significantly associated with long-term kidney outcomes. AKD stage 0C and stage 1 or greater are significantly associated with the primary outcome. Stage 1 or greater AKD presents higher risk than stage 0C AKD, with CKD incidence, progression, and KFRT occurring more frequently in higher AKD stages than mortality.

Accelerated versus standard initiation of renal replacement therapy for critically ill patients with acute kidney injury: a systematic review and meta-analysis of RCT studies

Summary: This study found that accelerated initiation of RRT can reduce the risk of all-cause mortality in surgical ICU patients and those receiving CRRT treatment, as well as increase the chances of eventual dialysis independence in these two subgroups. However, accelerated RRT initiation increases the risk of dialysis dependence in patients treated with non-CRRT methods or with high SOFA scores.
