Diagnostic performance of noncultural methods for central nervous system aspergillosis

Summary: This study aimed to explore the diagnostic performance of noncultural methods for central nervous system (CNS) aspergillosis. The combination of cerebrospinal fluid (CSF) galactomannan (GM) test and metagenomic next-generation sequencing (mNGS) showed the best diagnostic performance, with a sensitivity of 61.9% and specificity of 82.6%. Different manifestations of CNS aspergillosis were also investigated, and the lowest sensitivity was identified in the abscess group (42.9%), while the highest sensitivity was achieved in the abscess with encephalitis group (60.0%). For encephalitis patients with only CSF abnormalities, the combination test exhibited the best performance, with a sensitivity of 77.8% and specificity of 82.6%.

Non-fumigatus Aspergillus Infection Associated with a Negative Aspergillus Precipitin Test in Patients with Chronic Pulmonary Aspergillosis

Summary: Aspergillus antibody testing is crucial for diagnosing chronic pulmonary aspergillosis (CPA) with high sensitivity. However, false-negative results may occur depending on the Aspergillus species. This study identified non-fumigatus Aspergillus species as a significant factor associated with negative Aspergillus precipitin test results in CPA patients. The sensitivity of the test was found to be lower in non-fumigatus Aspergillus-associated CPA compared to A. fumigatus-associated CPA.

The clinical features and laboratory test performance vary among different subtypes. Low body mass index (BMI) is an independent risk factor for survival. Selective surgery and antifungal treatment for more than 6 months are associated with improved survival.

Performance of LDBio Aspergillus WB and ICT Antibody Detection in Chronic Pulmonary Aspergillosis

Summary: This study investigated the performance of Aspergillus antibody detection methods in 88 post-tuberculosis patients with suspected chronic pulmonary aspergillosis. Both Western blot and immunochromatography lateral flow detection showed good sensitivity and substantial agreement, with the combination of the two methods achieving the highest area under the curve result. The global intensity of Western blot correlated with the severity of symptoms in the chronic pulmonary aspergillosis group.
