Evaluating strength of recommendations for commonly administered medications in lactating women

Yevgenia Y. Fomina et al.

Summary: This study aimed to characterize the data on medications for lactating people in the LactMed database and evaluate the strength of the data for the most commonly administered medications in lactating women. The results showed that out of 1408 medications evaluated, 51% had no associated data, 47% had minimal-moderate data, and 2% had strong data.

Development of a Generic Physiologically-Based Pharmacokinetic Model for Lactation and Prediction of Maternal and Infant Exposure to Ondansetron via Breast Milk

Kathleen M. Job et al.

Summary: A physiologically-based pharmacokinetic lactation model for ondansetron was developed and validated, showing good agreement with observed data. The model predicted low relative infant doses, indicating that mothers receiving ondansetron treatment can safely breastfeed. The open-source model can be adapted to study other drugs and populations.

Levetiracetam in lactation: How much is excreted into human breast milk?

Natalie Dinavitser et al.

Summary: This study examined the excretion of levetiracetam in breastmilk in breastfeeding women with epilepsy, finding that exposure to the drug in breastmilk may lead to infant toxicities. The study also revealed a close correlation between levetiracetam dose, maternal serum concentration, and breastmilk levels. Further studies are needed to assess the short- and long-term safety of levetiracetam in breastfeeding infants.

Prediction of basic drug exposure in milk using a lactation model algorithm integrated within a physiologically based pharmacokinetic model

Amita Pansari et al.

Summary: Medication use during breastfeeding can potentially expose infants to drugs through breast milk. This study developed a physiologically based Pharmacokinetic Model (PBPK) to predict drug exposure in breast milk and estimate the potential infant dose. The model showed accurate predictions for the four drugs tested and can assist in evaluating the risk to infants from maternal exposure to these drugs.

Physiologically based pharmacokinetic model to predict drug concentrations of breast cancer resistance protein substrates in milk

Tao Zhang et al.

Summary: In this study, a lactation physiologically based pharmacokinetic (PBPK) modeling approach incorporating breast cancer resistance protein (BCRP) mediated transport kinetics was developed to predict the concentration-time profiles and milk-to-plasma (M/P) ratio of five drugs in nursing women's plasma and milk. The predictive performance of the PBPK models was evaluated and found to be accurate, indicating the usefulness of this approach in predicting drug pharmacokinetics in breast milk.

Summary: This study aimed to predict potential risks to neonates from maternal drug intake during lactation by developing a lactation PBPK model that considers physiological properties, drug ionization, and distribution, along with calculating infant doses.

Maternal Transfer of Cetirizine Into Human Milk

Hannah Wilkerson et al.

Summary: The study used high performance liquid chromatography mass spectrometry to analyze the concentration of cetirizine in human milk. The relative infant dose was calculated to be 1.77% at 24 hr, with no reported adverse effects in infants. This study suggests that the transfer of cetirizine into human milk is minimal and unlikely to pose a significant risk to breastfeeding infants.

Animal Models for In Vivo Lactation Studies: Anatomy, Physiology and Milk Compositions in the Most Used Non-Clinical Species: A Contribution from the ConcePTION Project

Domenico Ventrella et al.

Summary: Breastfeeding importance is widely recognized, but lack of scientific data on medication transfer during lactation causes pressure on women under treatment. ConcePTION project aims to provide reliable information on medication effects during pregnancy and breastfeeding. By summarizing lactation process features in animal species, the goal is to select an appropriate species for studying drug transfer through milk.

A comprehensive review on non-clinical methods to study transfer of medication into breast milk - A contribution from the ConcePTION project

Nina Nauwelaerts et al.

Summary: Breastfeeding is crucial for the health of both mother and infant, but there is a lack of information on the safety of maternal medication during breastfeeding. Non-clinical methods offer an attractive alternative to study the transfer of medication into human breast milk. The iterative development of in vitro, animal in vivo, and PBPK modelling methods may provide a promising approach to address this issue.

Umbilical Cord, Maternal Milk, and Breastfed Infant Levetiracetam Concentrations Monitoring at Delivery and during Early Postpartum Period

Ivana Kacirova et al.

Summary: The study found that the concentration of levetiracetam in maternal serum was similar to that in umbilical cord, higher in breastmilk compared to maternal serum, but significantly lower in infant serum. Breastfed infants had levetiracetam concentrations below the reference range used for the general population of epileptics.

Plasma and Breast Milk Pharmacokinetics of Tenofovir Disoproxil Fumarate in Nursing Mother with Chronic Hepatitis B-Infant Pairs

U. S. Erturk et al.

Summary: This study found that tenofovir concentration in maternal plasma was higher than breast milk and infant plasma, indicating a low passage of tenofovir disoproxil fumarate through breast milk. It suggests that tenofovir treatment is safe during breastfeeding in chronic hepatitis B patients.
