Spontaneous versus mechanical ventilation during video-assisted thoracoscopic surgery for spontaneous pneumothorax: A randomized trial

Jun Liu et al.

Summary: The study showed that spontaneous ventilation video-assisted thoracic surgery (SV-VATS) is not inferior to mechanical ventilation VATS (MV-VATS) in terms of complication rate for primary spontaneous pneumothorax patients undergoing blebectomy. Additionally, SV-VATS was associated with shorter procedural duration, recovery time, and food intake time.

Opioid Free Anesthesia in Thoracic Surgery: A Systematic Review and Meta Analysis

Filippo D'Amico et al.

Summary: This study conducted a systematic review and meta-analysis on the efficacy of opioid-free anesthesia in thoracic surgery. The results showed that compared to opioid-based anesthesia, opioid-free anesthesia can reduce postoperative complications, decrease morphine consumption, and provide better postoperative analgesia at 48 hours.

Thoracic surgery in the non-intubated spontaneously breathing patient

Matthias Grott et al.

Summary: Non-intubated video-assisted thoracic surgery (NIVATS) is a surgical technique performed on spontaneously breathing patients, which has advantages over one-lung-ventilation under general anaesthesia and is suitable for patients with interstitial lung diseases. However, prospective studies are still needed.

Local anaesthetic adjuncts for peripheral regional anaesthesia: a narrative review

N. Desai et al.

Summary: The administration of local anaesthetic adjuncts can extend the duration of peripheral nerve blocks, with dexmedetomidine and dexamethasone showing the best potential for improving block duration. Despite side effects and potential neurotoxicity, off-label use of systemic dexamethasone is recommended as a local anaesthetic adjunct for postoperative pain management, considering concerns related to nerve and muscle injury.

Summary: The study found that using dexmedetomidine for opioid-free balanced anesthesia does not reduce postoperative opioid-related adverse events, but instead leads to more serious adverse events, especially hypoxemia and vagal reflex.

Summary: A retrospective study found that NSCLC patients undergoing SV-VATS had significantly better long-term survival outcomes compared to those undergoing MV-VATS, with no significant differences in intraoperative and postoperative parameters.

Summary: The study retrospectively analyzed patients who underwent subxiphoid VATS resection for anterior mediastinal tumors between December 2015 and September 2019, dividing them into non-intubated VATS and intubated VATS groups. Results showed similar intraoperative and postoperative complications between the two groups, as well as comparable anesthesia time, operation time, chest tube duration, postoperative pain scores, and length of stay.

Perineural and intravenous dexamethasone and dexmedetomidine: network meta-analysis of adjunctive effects on supraclavicular bracheal plexus block

H. Sehmbi et al.

Summary: This network meta-analysis compared and ranked the effects of perineural and intravenous dexamethasone and dexmedetomidine as supraclavicular block adjuncts. The results showed that dexamethasone as an adjunct prolonged the durations of sensory and analgesic blockade to a greater extent than dexmedetomidine, regardless of route. Intravenous dexmedetomidine did not affect block characteristics.
