Chronic Pain in the Elderly: Mechanisms and Perspectives

Summary: Chronic pain in the elderly population is a current clinical challenge, as it leads to functional disability, coexisting psychological disorders, cognitive deficits, and sleep disturbances. The main painful complaints in the elderly are related to neurodegenerative and musculoskeletal conditions, peripheral vascular diseases, arthritis, and osteoarthritis, which negatively affect their quality of life, social participation, physical activity, and daily activities. Organ dysfunction and existing diseases significantly influence the perception and responses to chronic pain in this group. Elderly people exhibit altered pain experience, with changes in pain processing mechanisms, possibly due to the degeneration of pain modulating circuits. Aging is also associated with an increase in pain threshold, decline of painful sensations, and decrease in pain tolerance. Elderly patients with chronic pain are at an increased risk for dementia and cognitive impairment.

Evaluation of opioid consumption trends for pain in Taiwan and comparison with neighboring Asian countries

Summary: This study evaluated the trend of opioid consumption for pain management in Taiwan and compared it with neighboring Asian countries. The findings showed that opioid consumption in Taiwan has slowly increased from 2008 to 2018, with fentanyl and morphine being the most commonly used opioids. Compared with neighboring Asian countries, Taiwan's opioid consumption level was between Japan and Singapore.

Opiophobia in Emergency Department Healthcare Providers: A Survey in Western Switzerland

Summary: This study investigated the opiophobia among healthcare providers in EDs and its association with their personality traits. The results showed that doctors, less experienced individuals, and non-indigenous participants had higher scores in opiophobia, with fear of side effects and addiction being the major contributors. Additionally, the fear of side effects and addiction were associated with personality traits of healthcare providers in EDs.
