Summary: Plasma cfDNA levels were significantly elevated in COVID-19 patients compared to those with influenza or respiratory syncytial virus and healthy controls, with diverse tissue sources including hematopoietic cells, vascular endothelium, hepatocytes, adipocytes, kidney, heart, and lung. cfDNA levels correlated positively with COVID-19 severity, C-reactive protein, and D-dimer, and the cfDNA profile at admission could predict the need for intensive care or death during hospitalization. Increased cfDNA in COVID-19 patients led to excessive mitochondrial ROS production in renal tubular cells, which was inhibited by a TLR9-specific antagonist.

The physical and molecular features of cell-free DNA fragments, along with their distribution over the genome, contain information about their tissues of origin. These features, including DNA methylation patterns and nucleosomal organization, hold promise for noninvasive detection of tissue-specific pathologies with a variety of clinical applications.
