Rehabilitation for Total Knee Arthroplasty A Systematic Review

Summary: The study compared the benefits and harms of rehabilitation interventions for patients who underwent elective total knee arthroplasty for primary osteoarthritis. Evidence from 53 randomized controlled trials suggests that various rehabilitation programs after surgery may lead to similar improvements in pain, range of motion, and activities of daily living. Rehabilitation in the acute phase may increase strength, but postacute rehabilitation results in comparable strength. There is no evidence of harms from acute rehabilitation, and the risk of harms from postacute rehabilitation programs seems comparable. All findings have low strength of evidence.

Factors associated with persistent postsurgical pain after total knee or hip joint replacement: a systematic review and meta-analysis

This systematic review and meta-analysis aimed to identify these factors and analyze their impact on postoperative pain. Eighty-one studies involving 171,354 patients were included in the review, and it was found that factors such as state anxiety, depression, and preoperative pain scores were associated with an increased risk of postoperative pain. The findings highlight the need for further research in this area.

Epidemiology of osteoarthritis

Summary: This narrative review summarizes the current evidence on the prevalence, incidence, and risk factors of osteoarthritis (OA) at the person-level and joint-level. The review highlights the high prevalence of OA worldwide, with older individuals, women, certain racial and ethnic groups, and individuals with lower socioeconomic status being more affected. Obesity and joint injury are identified as modifiable risk factors with the strongest evidence. Other emerging topics with potential associations with OA risk or progression include specific vitamins and diets, high blood pressure, genetic factors, metformin use, bone mineral density, abnormal joint shape and malalignment, and lower muscle strength/quality. The review also emphasizes the heterogeneity of OA and the importance of understanding and defining OA phenotypes. Overall, OA is a prevalent condition with significant global impacts on health outcomes, necessitating efforts to mitigate obesity and joint injury risks.

Patient-Reported Mental Health Score Influences Physical Function After Primary Total Knee Arthroplasty

Summary: Patients with higher preoperative mental health scores had better preoperative and postoperative physical function scores, but those with the lowest mental health scores experienced a sharp decline in physical function approximately a year after surgery and did not recover. These findings suggest that poor mental health should not prevent TKA, but tighter follow-up and additional interventions may be necessary for patients with the lowest mental health scores.
