The role of preoperative venography in predicting the difficulty of a transvenous lead extraction procedure

Summary: We hypothesized that preoperative venography could predict the difficulty of transvenous lead extraction (TLE). A total of 105 patients with cardiac implantable electronic devices were included in this study. The results showed that moderate to severe stenosis, complete occlusion, and extensive lead-venous wall adherence were associated with the complexity of TLE. Patients with extensive lead-venous wall adherence required a longer fluoroscopy time and were more likely to require advanced extraction tools.

Disparities in transvenous lead extraction in young adults

Summary: This study assessed the safety and effectiveness of transvenous lead extraction (TLE) in young adults with cardiac implantable electronic devices (CIEDs). The results showed that young adults had longer implant duration and more severe lead adhesion, leading to higher procedure complexity and complications. Hemopericardium and tricuspid valve damage were more common in young adults compared to middle-aged and older adults.

Our study demonstrates high procedural efficacy and safety of TLE, with more challenging procedures observed in noninfective patients. Infection may potentially hinder the formation of fibrotic adhesions during TLE procedures.

Predictors of lead break during transvenous lead extraction

Results showed that high lead age, coradial bipolar leads, passive leads, and leads without polyurethane insulation were predictors of lead break, leading to longer procedure times and higher rates of cardiac tamponade.
