Effect of Antibiotic Exposure on Staphylococcus epidermidis Responsible for Catheter-Related Bacteremia

Summary: The aim of this study was to evaluate the impact of 15 days of antibiotic exposure at sub-inhibitory concentration on the resistance, fitness and genome evolution of Staphylococcus epidermidis strains responsible for catheter-related bacteremia. The results showed that antibiotic exposure affected bacterial pathogenicity and led to the emergence of resistant strains.

Blood bacterial resistant investigation collaborative system (BRICS) report: a national surveillance in China from 2014 to 2019

Summary: During a 6-year study period in China, Escherichia coli and Klebsiella pneumoniae were found to be the most common pathogens causing bloodstream infections. The prevalence of multidrug-resistant organisms declined or remained stable, except for carbapenem-resistant Klebsiella pneumoniae, which increased significantly. This poses a great therapeutic challenge for treating bloodstream infections.

Anaerobic bloodstream infections in Italy (ITANAEROBY): A 5-year retrospective nationwide survey

Summary: This study provides updated data on the burden and profile of anaerobic bloodstream infections (ABIs) in Italy. The findings show that the average age of ABIs patients is relatively high, with a majority being males. The susceptibility testing of antibiotics reveals a low resistance rate to metronidazole and a high resistance rate to penicillin among anaerobes. The resistance of Bacteroides spp. to clindamycin has increased over the study period. These results offer a comprehensive overview of the epidemiology of ABIs.

Multidrug-Resistant Acinetobacter baumannii May Cause Patients to Develop Polymicrobial Bloodstream Infection

Summary: Polymicrobial Acinetobacter baumannii bloodstream infections (AB-BSI), caused by multiple microorganisms, account for a significant proportion among all AB-BSI cases. Although it does not affect mortality, polymicrobial AB-BSI is associated with slightly longer hospital stays. Multidrug resistance is associated with the development of polymicrobial AB-BSI but does not directly cause it, while resistance to piperacillin/tazobactam is highly correlated with polymicrobial AB-BSI.

The majority of BSIs were caused by Gram-negative bacteria, particularly Escherichia coli and Klebsiella pneumoniae, which showed low susceptibility to several antibiotics. Hospital-acquired BSIs were associated with higher prevalence of resistant strains.

The results showed that the majority of patients with bacteremia had community-acquired disease, with a higher Sequential Organ Failure Assessment (SOFA) score being an independent predictor of poor 7-day clinical outcomes in patients with community-acquired H. influenzae bacteremia.

Serum Albumin Level as a Predictor of Outcome in Patients Admitted to Pediatric Intensive Care Units

Summary: The study found that hypoalbuminemia on admission is a good predictor of mortality in the PICU, with patients having lower albumin levels having a significantly higher mortality rate. Each unit increase in serum albumin was associated with a 28.9% decrease in the risk of mortality. Incorporating serum albumin with PRISM-III score improved the predictive value of mortality.

Systematic review and meta-analysis of the proportion and associated mortality of polymicrobial (vs monomicrobial) pulmonary and bloodstream infections by Acinetobacter baumannii complex

Summary: This study found that polymicrobial Acinetobacter baumannii complex (ABC) infections are common, with a pooled proportion of 27% of infections being polymicrobial. These infections were associated with lower 28-day mortality, but not with in-hospital mortality. However, due to considerable heterogeneity and limitations in defining polymicrobial infections, more research is needed to clarify the clinical impact of polymicrobial ABC infections.

Serum albumin and risks of hospitalization and death: Findings from the Atherosclerosis Risk in Communities study

Summary: This study aimed to determine whether lower serum albumin in community-dwelling, older adults is associated with increased risk of hospitalization and death independent of pre-existing disease. The results showed that lower baseline serum albumin concentration was significantly associated with higher risk of both hospitalization (incidence rate ratio [IRR]: 1.58) and death (hazard ratio [HR]: 1.67).

Summary: The study found that polymicrobial KP-BSI accounted for a significant proportion among all KP-BSI. Hospitalization in burn ward and intensive care unit was an independent risk factor for the development of polymicrobial KP-BSI. Patients with polymicrobial KP-BSI had a higher rate of carbapenem-resistant K. pneumoniae and might have poor outcomes compared to monomicrobial KP-BSI.

Predictors of mortality in adult patients with methicillin-resistant Staphylococcus aureus bloodstream infection: a meta-analysis and systematic review

Summary: Multiple factors are associated with mortality in patients with MRSA bloodstream infections, including severe sepsis or septic shock, congestive heart failure, liver cirrhosis, malignancy, infective endocarditis, and nosocomial infection. Timely surgical intervention, appropriate antimicrobial treatment, and assessment of disease severity based on the APACHE II score are important for improving patient outcomes in cases of MRSA bloodstream infections.
