Clinical and imaging evidence of brain-first and body-first Parkinson's disease

Jacob Horsager et al.

Summary: Braak's hypothesis has had a significant influence on Parkinson's disease research. However, a new model suggests that the existing model does not apply to all patients. By using REM-sleep behavior disorder as a clinical identifier, the disease can be divided into two subtypes: body-first PD and brain-first PD. These subtypes show differences in clinical symptoms and imaging features.

Dysautonomia and REM sleep behavior disorder contributions to progression of Parkinson's disease phenotypes

Giulietta Maria Riboldi et al.

Summary: Non-motor symptoms like dysautonomia and REM sleep behavior disorder in Parkinson's disease may indicate different types of the disease. Through analysis of data from newly diagnosed Parkinson's patients, it was found that dysautonomia symptoms can predict disease progression more accurately.

A Discrete Glycinergic Neuronal Population in the Ventromedial Medulla That Induces Muscle Atonia during REM Sleep and Cataplexy in Mice

Shuntaro Uchida et al.

Summary: This study identified a specific subpopulation of Gly(VMM) neurons in the ventromedial medulla that are involved in generating muscle atonia during REM sleep. Silencing this circuit resulted in REM sleep without muscle atonia and reduced time spent in cataplexy-like episodes. Additionally, Gly(VMM) neurons also play a crucial role in maintaining sleep and cataplexy.

Summary: The study found that approximately 33% of patients with Parkinson's disease had rapid eye movement sleep behavior disorder (RBD), and both motor and non-motor functions were significantly decreased in these patients, indicating a more extensive degeneration in patients with non-demented Parkinson's disease with RBD.

Sleep disturbance in movement disorders: insights, treatments and challenges

Grace A. Bailey et al.

Summary: Sleep and circadian rhythm disturbances are central features of many movement disorders, with implications for early disease detection. Medication for motor symptoms can impact sleep, highlighting the importance of addressing sleep disturbances for improved quality of life. Shared underlying mechanisms between motor and sleep pathophysiology warrant further investigation for better understanding and management of movement disorders.

Summary: Sleep disturbances are common nonmotor symptoms in patients with Parkinson's disease, including RBD, RLS, and EDS. Monitoring sleep problems, especially during the COVID-19 era, is crucial for improving patient's quality of life. Screening and managing sleep problems play an important role in clinical practice.
