Genetic, biochemical, and clinical spectrum of patients with mitochondrial trifunctional protein deficiency identified after the introduction of newborn screening in the Netherlands

Summary: Long-chain 3-hydroxyacyl-CoA dehydrogenase deficiency (LCHADD) is a disease included in newborn screening programs. Acylcarnitine-based screening not only detects LCHADD, but also other deficiencies of the mitochondrial trifunctional protein (MTP). This study retrospectively evaluated the characteristics and outcomes of MTP-deficient patients diagnosed in the Netherlands since 2007. The results showed that patient outcomes were highly variable, emphasizing the need for accurate classification and discrimination of MTP deficiencies.

Summary: Long-chain 3-hydroxyacyl-CoA deficiency (LCHADD) and mitochondrial trifunctional protein (MTPD) are a group of inherited metabolic diseases that affect the degradation of long-chain fatty acids. Recent studies have shown that LCHADD patients display disease-specific alterations in complex lipids. In addition to the accumulation of acylcarnitines, dysregulation of complex lipids has been associated with other neurodegenerative diseases.

Whole-exome sequencing identified two novel heterozygous mutations in the HADHB gene. Symptom alleviation was effectively achieved through dietary restrictions. This study expands the clinical spectrum of disorders caused by HADHB mutations and suggests the effectiveness of dietary treatment is related to age of onset and symptom severity.
