Coronary artery plaque rupture and erosion: Role of wall shear stress profiling and biological patterns in acute coronary syndromes

Giulio Russo et al.

Summary: This study aimed to provide a comprehensive description of haemodynamic and biological characteristics of unstable and stable coronary artery plaques and investigate the correlation between wall shear stress (WSS) and molecular pathways. The results showed that nine genes were significantly overexpressed in unstable patients compared to stable patients, with four genes displaying different levels between intact fibrous cap (IFC) and ruptured fibrous cap (RFC) groups. Additionally, higher WSS was observed in the RFC group and significant correlations were found between certain genes and WSS values in the RFC group. These findings suggest the importance of WSS profiling in characterizing plaque instability.

Wall Shear Stress Associated with Stroke Occurrence and Mechanisms in Middle Cerebral Artery Atherosclerosis

Ho Geol Woo et al.

Summary: This study compared the differences in plaque characteristics and hemodynamic parameters in patients with MCA atherosclerosis according to stroke mechanism. It found that stroke caused by artery-to-artery embolism was associated with plaque enhancement and the highest maximum wall shear stress (WSS), and had a greater variability in maximum WSS.

The Correlation Between Wall Shear Stress and Plaque Composition in Advanced Human Carotid Atherosclerosis

A. M. Moerman et al.

Summary: This study investigated the relationship between wall shear stress (WSS) and plaque composition in advanced atherosclerosis. The findings revealed that high time-averaged WSS (TAWSS) is associated with larger macrophage areas and necrotic core sizes, while low oscillatory shear index (OSI) is associated with larger macrophage areas. Moreover, a combination of low TAWSS and low OSI is related to larger cap thickness.

Evaluation of Carotid Plaque Rupture and Neovascularization by Contrast-Enhanced Ultrasound Imaging: an Exploratory Study Based on Histopathology

Qi Lyu et al.

Summary: The study found that contrast agent inside-out direction diffusion is an important indicator for detecting vulnerable plaque fibrous cap rupture, with the degree of enhancement correlating with the morphology of intraplaque neovascularization. Increasing plaque enhancement was associated with higher microvessel density, larger microvessel area, and more irregular-shaped microvessels, indicating an increased risk of plaque vulnerability.

Advances in Multimodality Carotid Plaque Imaging: AJR Expert Panel Narrative Review

Daniel Bos et al.

Summary: Contemporary imaging methods have allowed for detailed visualization of carotid atherosclerotic plaque, which has led to significant advancements in in vivo evaluation of carotid plaque imaging. While traditional ultrasound-based assessment of luminal stenosis has historically been used to determine ischemic stroke risk, newer imaging techniques such as CT and MRI have shown that specific characteristics of vulnerable plaque may be more indicative of stroke risk. This review highlights six promising imaging-based plaque characteristics that provide unique information on plaque vulnerability and their potential impact on risk of ischemic stroke.

Summary: Atherosclerosis initiation is related to abnormal hemodynamic parameters at arterial bifurcations, with severe stenosis causing significant localized changes in hemodynamic parameters. Mild stenosis has minor impact on hemodynamic parameters.

Summary: This study found that total cholesterol, non-high-density lipoprotein cholesterol, and low-density lipoprotein cholesterol were positively associated with the presence of carotid IPN in a high-stroke-risk Chinese population. The findings also suggest that non-high-density lipoprotein cholesterol remained significantly associated with IPN presence even after adjusting for low-density lipoprotein cholesterol.

Summary: The study highlights the correlations between plaque morphology and biomechanical environment in arterial remodeling, suggesting a dynamic interaction between mechanical stresses and plaque progression. Plaque heterogeneity is found to be associated with plaque progression, and can be combined with mechanical stresses to identify high-risk plaques.

Plaque Elasticity and Intraplaque Neovascularisation on Carotid Artery Ultrasound: A Comparative Histological Study

Ying Zhang et al.

Summary: This study investigated the relationship between intraplaque neovascularisation and plaque elasticity using contrast enhanced ultrasound (CEUS) and shear wave elastography (SWE). The findings showed a negative correlation between intraplaque neovascularisation and elasticity, suggesting that more neovascularisation leads to lower elasticity in the plaque. The histopathological findings further validated this correlation.

Summary: A novel phantom platform of neovascularized atherosclerotic plaque in the carotid artery was developed in this study, utilizing synthetic plaque models and commercially available technology for quantitative measurement of IPN.
