Characterising within-hospitalSARS-CoV-2 transmission events using epidemiological and viral genomic data across two pandemic waves

Benjamin B. Lindsey et al.

Summary: This study combines epidemiological and viral genomic data to characterize the transmission of SARS-CoV-2 within a UK hospital. The results show that control measures implemented during the pandemic have reduced staff-to-staff transmission, but have been insufficient to prevent increasing patient-to-patient transmission. Therefore, earlier identification and control of hospital-acquired cases are necessary to break the transmission chains within hospitals.

Barriers to physical distancing among healthcare workers on an academic hospital unit during the coronavirus disease 2019 (COVID-19) pandemic

Sara C. Keller et al.

Summary: This qualitative study observed healthcare workers' activities and interactions in a medical unit, identifying high-risk interactions during patient handoffs and rounds. Barriers to physical distancing included computer workstation availability and the need to communicate confidential patient information. Recommendations to improve physical distancing in hospitals include restructuring workstations, using visible cognitive aids, adjusting shift times, and supporting virtual meetings. Additional strategies to promote staff adherence to physical distancing include rewarding positive behaviors and providing safe avenues for social connection.

Risk of Healthcare-Associated Transmission of Sever Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) in Hospitalized Cancer Patients

Karin Chow et al.

Summary: The risk of hospital-acquired COVID-19 is low for high-risk hospitalized patients, especially during periods of low community prevalence. However, shared occupancy with an infected patient can lead to a high secondary infection rate in roommates. Routine testing is unnecessary when community rates are low.

Combined epidemiological and genomic analysis of nosocomial SARS-CoV-2 infection early in the pandemic and the role of unidentified cases in transmission

Luke B. Snell et al.

Summary: Analysis of viral genome sequences and epidemiological data in a large healthcare institution during the early stages of the COVID-19 pandemic revealed 44 transmission clusters, with genomics playing a crucial role in accurately resolving these clusters. Evidence suggests that unidentified cases, such as healthcare workers or asymptomatic patients, may be important vectors of transmission.

Occupational and community risk of SARS-CoV-2 infection among employees of a long-term care facility: an observational study

Lauriane Lenggenhager et al.

Summary: This study investigated the contribution of occupational and community exposure to COVID-19 risk among healthcare workers in a university-affiliated long-term care facility. The results suggest that both occupational and community exposures play a significant role in the transmission of SARS-CoV-2.

Investigation of healthcare-associated COVID-19 in a large French hospital group by whole-genome sequencing

Valentin Leducq et al.

Summary: This study aimed to estimate the frequency and impact of healthcare-associated COVID-19 (HA-COVID-19) and evaluate the contribution of whole-genome sequencing (WGS) in cluster investigation. The results showed a close correlation between the incidence of HA-COVID-19, community-associated COVID-19 (CA-COVID-19), and COVID-19 among healthcare workers (HCWs). WGS analysis helped identify and exclude cases initially assigned to a cluster.

Whole-genome Sequencing to Track Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) Transmission in Nosocomial Outbreaks

Mary Lucey et al.

Summary: This study utilized whole virus genome sequencing to analyze hospital-acquired COVID-19 cases, revealing that transmission primarily occurred among patients dependent on nursing care through close contact or droplet transmission. The majority of cases were transmitted from healthcare workers to patients, highlighting the importance of interventions like comprehensive screening and universal use of surgical masks to prevent viral spread in healthcare settings. Collaboration between guidance bodies and IPC experts is crucial for developing effective control measures in emergency pandemic situations.

Severe Acute Respiratory Syndrome Coronavirus-2 Infections in Critical Care Staff: Beware the Risks Beyond the Bedside

Kate El Bouzidi et al.

Summary: In a large critical care department in London, researchers found that healthcare workers were less likely to contract SARS-CoV-2 from patients in critical care and were more likely to be infected by other staff, areas of the hospital, or the wider community. This indicates that personal protective equipment was effective in preventing transmission from patients. Additionally, staff need to maintain protective measures away from the bedside.

Estimating the burden of COVID-19 on the Australian healthcare workers and health system during the first six months of the pandemic

Ashley L. Quigley et al.

Summary: This study found that healthcare workers in Australia were at nearly 3 times the risk of contracting COVID-19 compared to the general community, with a HCW infection rate of 90/100000. Nosocomial outbreaks had significant impact on workforce capacity, leading to quarantine of numerous HCW. The study highlights the importance of improving PPE policies and protocols to protect healthcare workers during outbreaks.

SARS-CoV-2 antibody prevalence in England following the first peak of the pandemic

Helen Ward et al.

Summary: The REACT-2 study in England found that by mid-July 2020, around 6% of adults had been infected with SARS-CoV-2, with healthcare workers and individuals of Black or South Asian ethnicity disproportionately affected. Although different ethnicities had similar infection fatality ratios, higher hospitalization and mortality rates in minority ethnic groups may be due to higher rates of infection rather than differential disease experience or care.

SARS-CoV-2 outbreak in a tri-national urban area is dominated by a B.1 lineage variant linked to a mass gathering event

Madlen Stange et al.

Summary: This study investigated the early phase of the COVID-19 outbreak in Basel, Switzerland, and found evidence of limited community spread from travelers and family transmissions, as well as extensive cryptic community spread linked to a mass gathering event in the region. By combining whole-genome sequencing and phylogenetic analyses, the study was able to trace the transmission routes and evolution of viral variants during the outbreak.

Summary: The study utilized deep meta-transcriptomic sequencing to identify SNP types and dynamics of SARS-CoV-2 mutations, revealing viral evolution during infection and independent compartmentalization of populations in different respiratory tissues. Positive selection amino acids and high-frequency alternate allele variants were observed in the virus genome.
