The GIP/GIPR axis in medullary thyroid cancer: clinical and molecular findings

Summary: This study explores the expression and clinical significance of GIPR in endocrine tumors, particularly in MTC. It finds that GIPR is expressed in around 80% of MTC tumor specimens, especially in sporadic, larger, and advanced-stage cancers. GIPR stimulation leads to increased cAMP levels and fluctuation in calcium ion in MTC-derived cells, potentially associated with increased calcitonin secretion. However, further research is needed to understand the molecular mechanisms and clinical implications of GIP/GIPR axis activation in MTC.

Paradoxical GH increase during oral glucose load may predict overall remission in acromegalic patients

Impairment in insulin secretion without changes in insulin resistance explains hyperglycemia in patients with acromegaly treated with pasireotide LAR

Summary: Pasireotide, a second-generation somatostatin receptor ligand (SRL) used for treating acromegaly, may be associated with glucose metabolism impairment. In a retrospective study of 33 patients, we observed that treatment with pasireotide was associated with normalization of serum IGF-1 in almost 60% of patients, but one-third of patients developed diabetes. In the patients who stopped pasireotide because of hyperglycemia, HbA1c promptly decreased. Longitudinal data in 14 patients show that diabetes is mediated by impaired insulin secretion, which occurred shortly and then remained stable on long term, while no significant changes in insulin sensitivity were observed, despite a marked reduction of GH/IGF-1 concentrations. Older age and a worse glycemic control at baseline were the strongest predictors for hyperglycemia.

Clinical presentation and management of acromegaly in elderly patients

Summary: Elderly patients with acromegaly are not uncommon, and primary medical therapy is a reasonable option with effective results, while the rate of surgical success is not reduced. A careful cost-benefit balance is suggested. Disease-specific comorbidities are more prevalent in acromegalic patients with a longer follow-up rather than in those diagnosed aged >= 65 years.

Etiology and outcome of acromegaly in patients with a paradoxical GH response to glucose

Summary: Recent studies have focused on somatotroph pituitary adenomas in acromegaly patients showing a paradoxical GH response during OGTT, which has been linked to the ectopic expression of GIP receptor in adenoma cells. These adenomas are negative for activating GNAS gene mutations and have unique cytogenetic and DNA methylation profiles. Patients with paradoxical GH response pattern have higher IGF-1 levels and smaller adenomas, often densely granulated, and may respond better to somatostatin receptor ligands. Targeted therapy against GIP receptor on these adenomas could be a new treatment approach for these patients.
