Acute myeloid leukemia with cup-like blasts and FLT3-ITD and NPM1 mutations mimics features of acute promyelocytic leukemia: a case of durable remission after sorafenib and low-dose cytarabine

Jie Sun et al.

Summary: This study reports a rare subtype of AML that resembles APL in terms of nuclear morphology but carries FLT3 and NPM1 mutations. The patient showed no response to standard treatment but achieved durable complete remission with sorafenib combined with LDAC therapy.

Summary: APL, a subtype of AML, has a favorable prognosis thanks to ATRA and ATO combination therapy, but some cases caused by atypical transcripts have variable response rates to treatment, posing a challenge for clinical management.

Diagnosis and management of AML in adults: 2022 recommendations from an international expert panel on behalf of the ELN

Hartmut Doehner et al.

Summary: The 2010 and 2017 editions of the European LeukemiaNet recommendations for AML diagnosis and management are widely recognized. There have been major advances in our understanding of AML, including disease classification updates, improvements in genomic diagnostics, and the development of new therapeutic agents. This update includes a revised genetic risk classification, response criteria, and treatment recommendations.

Acute promyelocytic leukemia: Immunophenotype and differential diagnosis by flow cytometry

Hong Fang et al.

Summary: This study characterized the immunophenotype of APL and its mimics using flow cytometric analysis. APL-D showed distinctive immunophenotypic features, including positivity for CD2, CD34, CD13, CD64, and MPO, which can distinguish it from other AML subtypes.

A short report of novel RARG-HNRNPM fusion gene in resembling acute promyelocytic leukemia

Yang Song et al.

Summary: Background resembling acute promyelocytic leukemia (APL) is a unique subtype of APL with different features, often unresponsive to arsenic trioxide and all-trans retinoic acid. This study reported a novel fusion gene RARG-HNRNPM in a 25-year-old female patient using RNA-seq. Combined chemotherapy regimen showed beneficial effects in treating this patient.

Acute myeloid leukemia with mutated NPM1 mimics acute promyelocytic leukemia presentation

Manuel J. Arana Rosainz et al.

Summary: This study investigated the clinicopathological and molecular features of NPM1-mutated AML with infrequent APL-like phenotype. It found an association between NPM1-mutated AML with TET2 or IDH2 co-mutations and APL-like immunophenotype. Additionally, significant elevated D-dimer levels or overt DIC at diagnosis could be relevant for this AML subtype.

Quantification of NG2-positivity for the precise prediction of KMT2A gene rearrangements in childhood acute leukemia

Elena Zerkalenkova et al.

Summary: The study found a significant increase in NG2 expression in AL patients with KMT2A rearrangements, with defined threshold levels for NG2-positivity established through ROC analysis. Improved diagnostic performance was observed with the defined threshold levels compared to conventional thresholds.

Current views on the genetic landscape and management of variant acute promyelocytic leukemia

Xiang Zhang et al.

Summary: Acute promyelocytic leukemia (APL) consists of typical APL and variant APL, with the latter showing distinct features and challenges in recognition, diagnosis, and treatment. Genetic studies have revealed a great heterogeneity in the genetics of variant APL, with different rearrangements other than RARA/RARB/RARG being able to drive the disease. Treatment for variant APL often involves combined chemotherapy instead of differentiation induction therapy.
