Association of ITPA gene polymorphisms with adverse effects of AZA/6-MP administration: a systematic review and meta-analysis

Summary: In this study, a systematic review and meta-analysis were conducted to investigate the genetic association of ITPA polymorphisms with adverse effects in patients treated with AZA/6-MP. The results showed that rs1127354 and rs7270101 were associated with neutropenia and other adverse effects in different populations. Stratification analysis also confirmed the association between neutropenia and these two polymorphisms in ALL patients. These findings suggest that ITPA polymorphisms could serve as predictive biomarkers for adverse effects of thiopurine drugs, providing personalized treatment options for ALL patients.

Comprehensive analysis of dose intensity of acute lymphoblastic leukemia chemotherapy

Summary: This study analyzed the dosages of chemotherapy drugs in two acute lymphoblastic leukemia trials. The results showed that, compared to pegaspargase, the native E. coli L-asparaginase formulation had higher dose intensities for some drugs. Additionally, patients with lower dose intensities of asparaginase had higher dose intensities of mercaptopurine. However, lower chemotherapy dose intensity was not associated with relapse.

Association Between Genetic Polymorphisms of Metabolic Enzymes and Azathioprine-Induced Myelosuppression in 1,419 Chinese Patients: A Retrospective Study

Summary: This study found that the NUDT15 (415C>T) gene polymorphism may play a role in the adverse effects of myelosuppression induced by azathioprine in the Chinese population, and there is an epistatic interaction between ITPA (94C>A) and NUDT15 (415C>T). It is recommended to check the relevant genotypes before administration and avoid the use of azathioprine in patients carrying a homozygous NUDT15 (415C>T) mutation.

TPMT*3C as a Predictor of 6-Mercaptopurine-Induced Myelotoxicity in Thai Children with Acute Lymphoblastic Leukemia

Summary: This study investigated the association between genetic polymorphisms of drug-metabolizing enzymes and drug transporters with 6-MP-related myelotoxicity and hepatotoxicity in Thai children with ALL. The results showed that children with TPMT*1/*3C genotype were at higher risk of leukopenia, while heterozygous TPMT*3C was significantly associated with severe neutropenia. No association was found between ITPA and MRP4 gene polymorphisms with myelotoxicity and hepatotoxicity. The findings emphasize the importance of genotyping the TPMT gene before initiating 6-MP therapy to assess the risk of myelotoxicity.
