Tracheostomy Decannulation and Disorders of Consciousness Evolution

Pablo A. Bellon et al.

Summary: This study indicates that patients who show improvement in disorders of consciousness are more likely to have successful tracheostomy decannulation. Additionally, some patients in a vegetative state were able to be decannulated with lower mortality rates compared to those who remained tracheostomized.

Brain Data in Pediatric Disorders of Consciousness: Special Considerations

Fatima Y. Ismail et al.

Summary: The diagnosis and management of disorders of consciousness in children pose a challenge in clinical, research, and ethical aspects. Current guidelines for adults lack similar recommendations for infants and children due to their distinct conscious experience and limited behavioral repertoire. Additionally, the developing brain in infants and children undergoes heightened structural and functional neuroplasticity, adding complexity to the investigation of neural correlates of consciousness. This review discusses the clinical assessment of pediatric disorders of consciousness, highlighting the diagnostic and prognostic value of neurophysiological and neuroimaging correlates of consciousness and their potential relevance based on existing theoretical models of consciousness in adults.

Behavioral Assessment of Patients With Disorders of Consciousness

Eileen Fitzpatrick-DeSalme et al.

Summary: Brain injury can lead to coma and result in prolonged disorder of consciousness, including vegetative and minimally conscious states. Early detection of emerging consciousness is important for prognosis, and improvement in consciousness indicates potential for communication and environmental control. However, assessing consciousness accurately is challenging due to various factors. This article discusses strategies for optimizing patient status during assessment and reviews different assessment approaches.

Classifying Disorders of Consciousness: Past, Present, and Future

Daniel Kondziella et al.

Summary: With advanced analytical methods applied to neuroimaging and neurophysiological data, cerebral conditions that challenge the established classification of disorders of consciousness have been identified. These conditions, known as cognitive motor dissociation, involve brain-damaged patients who cannot perform motor commands but exhibit patterns of cerebral activation similar to healthy controls during mental imagery tasks. The future classification of impaired consciousness and responsiveness will be influenced by the availability of data to characterize brain states, the modeling paradigms used for data analysis, and the implementation of classification schemes in clinical settings.

Applications of Advanced MRI to Disorders of Consciousness

David Fischer et al.

Summary: Disorders of consciousness (DoC) after severe brain injury are challenging for clinicians due to uncertainties in diagnosis, prognosis, and management. Advanced MRI technology, including diffusion MRI and functional MRI, has the potential to improve understanding of DoC pathophysiology, aid in identifying patient consciousness, and enhance clinical prognostication. This article reviews emerging evidence for the application of advanced MRI in patients with DoC.

Empowering the voiceless. Disorders of consciousness, neuroimaging and supported decision-making

Timo Istace

Summary: This article discusses the possibility of communicating with patients suffering from impaired consciousness in order to obtain their informed consent. It first presents the current state-of-the-art in neuroimaging research that shows the potential for communication with some of these patients. It then explores how obtaining informed consent from these patients is possible, considering the specificities and limitations of communication via neuroimaging. Lastly, it analyzes the ethical and legal considerations that need to be addressed before introducing neuroimaging as a means to obtain informed consent, and examines the concept of supported decision-making and its relevance to the use of neurotechnology in supporting minimally conscious patients' ability to make decisions about their own medical treatment.

Neuromodulation Applied to Diseases: The Case of HRV Biofeedback

Asaf Gitler et al.

Summary: This article narratively reviews the biological mechanisms underlying the role of vagal activity and vagally mediated heart rate variability in various health conditions. The study found that HRV-biofeedback, a technique that combines slow-paced breathing and visual feedback of HRV, can improve clinical outcomes in coronary heart disease, pain, and hypertension. However, evidence for its effectiveness in diabetes and dementia is limited.

Behavioral Assessment of Patients with Disorders of Consciousness

Yelena G. Bodien et al.

Summary: Assessing the level of consciousness in patients with disorders of consciousness is critical for diagnosis and treatment evaluation. While standardized behavioral assessment is recommended over routine evaluation, it is still susceptible to biases and misdiagnosis. Factors that may confound the examination should be considered.

Xinnan Li et al.

Summary: This study used double diffusion encoding MRI to detect brain changes induced by cognitive training. The results showed that mu FA in the left middle frontal gyrus decreased after training and was moderately correlated with changes in response time in the orienting component of attention. No significant changes were observed for other imaging indices.

Summary: The study reported a 61-year-old female who experienced left hemiparesis following an initial stroke and again after a left subdural hematoma. Through fMRI and DTI scans, it was found that cortical reorganization may have occurred in the brain, with the left hemisphere taking over the movement of the left side of the body.

Update on neuroimaging in disorders of consciousness

Leandro R. D. Sanz et al.

Summary: Neuroimaging has become an important tool in assessing disorders of consciousness, with technologies like magnetic resonance imaging, high-density electroencephalography, and positron emission tomography providing valuable information on brain connectivity, language function, covert consciousness detection, and prognostic markers. A multimodal approach is recommended to assess consciousness, with a focus on integrating different techniques to provide comprehensive diagnostic and prognostic information. Methods like transcranial magnetic stimulation and near-infrared spectroscopy show promise in clinical applications for assessing consciousness in severely brain-injured patients.
