Pleurectomy Decortication in the Treatment of Malignant Pleural Mesothelioma Encouraging Results and Novel Prognostic Implications Based on Experience in 355 Consecutive Patients

Summary: This study retrospectively analyzed the clinical data and treatment outcomes of 355 consecutive patients who underwent PDC surgery in a single institution over a period of 9 years. The results showed that PDC surgery had good treatment efficacy, low mortality and manageable complication rates. The 5-year survival rate of patients undergoing PDC with MCR in a multi-modality setting was close to 25%, depending on quantitative and clinical stage, sex and histological subtype.

The perioperative morbidity was also lower in the surgery group.

Systematic Review and Meta-Analysis of Pleurectomy/Decortication versus Extrapleural Pneumonectomy in the Treatment of Malignant Pleural Mesothelioma

Summary: This study compared the overall survival and postoperative complications of extrapleural pneumonectomy (EPP) and pleurectomy decortication (P/D) in the treatment of malignant pleural mesothelioma (MPM) through a systematic review of the available literature. The results showed that patients undergoing P/D had better overall survival and lower 30-day mortality, but no significant difference was found in 90-day mortality. The incidence of postoperative complications, such as empyema, atrial fibrillation, bleeding, and bronchopleural fistula, was significantly higher in the EPP group, while prolonged air leakage was only characteristic of P/D.
