Whole-muscle fat analysis identifies distal muscle end as disease initiation site in facioscapulohumeral muscular dystrophy

Summary: By assessing fat infiltration along the full proximo-distal axis of lower extremity muscles using quantitative MRI, this study found that the initiation site of facioscapulohumeral dystrophy (FSHD) commonly starts at the distal end of affected muscles and progresses in a wave-like manner towards the proximal end. Inflammatory lesions in the early phases of the disease preferentially occur at the distal muscle end. Whole-muscle fat assessment is essential for properly diagnosing FSHD and monitoring its progression.

Global versus individual muscle segmentation to assess quantitative MRI-based fat fraction changes in neuromuscular diseases

Summary: In this study, different segmentation approaches including global muscle segmentation were compared for evaluating disease progression in various neuromuscular disorders. The global muscle segmentation showed high sensitivity to change in fat fraction in most investigated disorders. Compared to individual muscle drawing, global muscle segmentation was found to be a faster and easier approach to assess disease progression, while individual muscle ROIs are still important for exploring selective muscle involvement.

Longitudinal study of MRI and functional outcome measures in facioscapulohumeral muscular dystrophy

Summary: This study found that muscles with intermediate baseline fat fraction were more likely to progress in FSHD patients. Functional outcome measures were correlated with overall MRI disease burden but did not show significant changes in the short term. Quantitative strength measurement of the TA revealed a steep loss of strength when the muscle had more than 20% fatty infiltration.
