Early outcomes of transcatheter versus surgical aortic valve implantation in patients with bicuspid aortic valve stenosis

Monil Majmundar et al.

Summary: This study compared early outcomes between transcatheter aortic valve implantation (TAVI) and surgical aortic valve replacement (SAVR) in patients with bicuspid aortic valve (BAV) stenosis. In the propensity score-matched cohort, TAVI was associated with reduced odds of in-hospital mortality and a similar risk of 30-day and six-month major adverse cardiovascular events (MACE), supporting the feasibility of TAVI in BAV patients without a need for concurrent aortic root repair.

Cerebral Embolic Protection during Transcatheter Aortic-Valve Replacement

Samir R. R. Kapadia et al.

Summary: Among patients with aortic stenosis undergoing transfemoral TAVR, the use of CEP did not have a significant effect on the incidence of periprocedural stroke, but on the basis of the 95% confidence interval around this outcome, the results may not rule out a benefit of CEP during TAVR.

Self-expanding Transcatheter vs Surgical Aortic Valve Replacement in Intermediate-Risk Patients 5-Year Outcomes of the SURTAVI Randomized Clinical Trial

Nicolas M. Van Mieghem et al.

Summary: Among intermediate-risk patients with symptomatic severe aortic stenosis, major clinical outcomes at 5 years were similar for TAVR and surgery. TAVR was associated with superior hemodynamic valve performance but also with more paravalvular leak and valve reinterventions.

Outcomes After Transcatheter Aortic Valve Replacement in Bicuspid Versus Tricuspid Anatomy A Systematic Review and Meta-Analysis

Claudio Montalto et al.

Summary: The study aimed to compare TAVR in BAV vs TAV stenosis. It found that TAVR is feasible in selected BAV patients, but caution is needed due to higher rates of PVL, annular rupture, and cerebral ischemic events in the BAV group.

Silent brain infarcts and early cognitive outcomes after transcatheter aortic valve implantation: a systematic review and meta-analysis

Kei Woldendorp et al.

Summary: Silent brain infarcts are common after TAVI, with diabetes, kidney disease, and pre-dilation increasing the overall SBI risk. Higher numbers of new SBIs appear to adversely affect early neurocognitive outcomes.

Heterogeneity of debris captured by cerebral embolic protection filters during TAVI

Herbert Kroon et al.

Summary: This study aimed to investigate the debris captured during transcatheter valve implantation, with findings showing differences in debris composition among different valve types. The Lotus valve was associated with a higher frequency of capturing foreign body materials, endothelium, calcified material, and myocardial tissue compared to other valve types. Repositioning of the valve and certain valve types were found to be risk factors for dislodging larger particles to the brain.

Trends in Cerebral Embolic Protection Device Use and Association With Stroke Following Transcatheter Aortic Valve Implantation

Dhaval Kolte et al.

Summary: This study analyzed the association between cerebral embolic protection device (CEPD) use and stroke following transcatheter aortic valve implantation (TAVI). The findings showed that there were no significant differences in in-hospital stroke or transient ischemic attack (TIA) rates between TAVIs with CEPD and TAVIs without CEPD. The study also observed an increase in CEPD utilization and the proportion of TAVI hospitals using CEPD during the study period.
