Bacterial overgrowth and lactose intolerance: how to best assess

Summary: This review provides an update on the clinical assessment of two important gastrointestinal problems, namely small intestinal bacterial overgrowth (SIBO) and lactose intolerance. The assessment of SIBO predominantly involves hydrogen/methane breath tests using glucose as a substrate and/or small bowel aspirate and culture, but these tests have limitations. Similarly, the assessment of lactose intolerance can be done through hydrogen/methane breath tests using lactose as a substrate, or through small bowel mucosal lactase assay, genetic testing, and lactose tolerance test. However, the efficacy and practicality of these diagnostic modalities vary. These conditions should be considered in patients with gastrointestinal symptoms that are unexplained by routine tests.

Summary: This study combines breath testing and stool microbiome sequencing to identify distinct gut microtypes associated with IBS-C and IBS-D subtypes. The findings suggest that methanogens such as M. smithii and H2S producers such as Fusobacterium and Desulfovibrio spp play a role in driving these subtypes.

Small Intestinal Bacterial Overgrowth in Functional Dyspepsia: A Systematic Review and Meta-Analysis

Summary: This systematic review and meta-analysis found a significant association between functional dyspepsia (FD) and small intestinal bacterial overgrowth (SIBO), with SIBO prevalence higher in FD patients compared to controls. The quality of evidence was low due to heterogeneity in breath test types and clinical factors. Further well-designed studies are needed to confirm the link between SIBO and FD.
