Summary: Residents of urban slums in Salvador, Brazil, are at high risk for rat-borne leptospirosis due to individual, socioeconomic, and environmental factors. A study conducted in four urban slums found that exposure to environmental sources of contamination, rather than rat reservoirs, contributes to higher transmission risk in areas with inadequate sanitation. Poorer residents may be exposed away from their households, and ongoing governmental interventions have not been shown to lower transmission risk for these vulnerable populations.
