A systems thinking approach for antimicrobial stewardship in primary care

Sajal K. Saha et al.

Summary: Although key AMS strategies are known, effective implementation of AMS programs in primary care is hindered by unclear health system components. Systems thinking is crucial in designing and implementing AMS programs, with a systemigram potentially being a useful tool to facilitate implementation.

Point-of-care C-reactive protein testing service for respiratory tract infections in community pharmacy: a qualitative study of service uptake and experience of pharmacists

Petra Czarniak et al.

Summary: Based on interviews with Australian pharmacists, targeted intervention measures such as point-of-care C-reactive protein testing have the potential to reduce inappropriate antimicrobial use, and pharmacists play an important role in this regard. The service was found to be simple, fast, reliable, and accurate, aiding clinical decision-making and antimicrobial stewardship. Accessibility and credibility of pharmacists were major facilitators for service provision and uptake, while time constraints and heavy documentation were identified as barriers.

The Role of Point-of-Care C-Reactive Protein Testing in Antibiotic Prescribing for Respiratory Tract Infections: A Survey among Swiss General Practitioners

Nahara Anani Martinez-Gonzalez et al.

Summary: Research found that GPs rely heavily on C-reactive protein point-of-care testing (CRP-POCT) in making antibiotic prescribing decisions, showing a preference for using lower CRP cut-offs to guide prescribing for more severe respiratory tract infections. However, patient pressure, diagnostic uncertainty, fear of complications, and lack of understanding of antibiotic resistance are reported as the major barriers to appropriate antibiotic prescribing.

A Novel GPPAS Model: Guiding the Implementation of Antimicrobial Stewardship in Primary Care Utilising Collaboration between General Practitioners and Community Pharmacists

Sajal K. Saha et al.

Summary: This study aims to implement antimicrobial stewardship programs through interprofessional collaboration between GPs and CPs. Using various research methods, the study generated secondary and primary evidence, and designed a prototype GPPAS model using a Systems Engineering Initiative for Patient Safety framework. The GPPAS model serves as a global guide for optimizing antimicrobial use in primary care through improved collaboration between GPs and CPs.

Antimicrobial stewardship by Australian community pharmacists: Uptake, collaboration, challenges, and needs

Sajal K. Saha et al.

Summary: Community pharmacists are aware of the importance of appropriate antibiotic use but lack training and resources for antimicrobial stewardship activities. Improving GP receptiveness and establishing system structures for increased GP-pharmacist collaboration appear to be a priority to accelerate pharmacist-led antimicrobial stewardship implementation. Further research is needed to understand stakeholders' views on the feasibility of implementing evidence-based GP-pharmacist collaborative AMS approaches.
