A dosing nomograph for cerebrospinal fluid penetration of meropenem applied by continuous infusion in patients with nosocomial ventriculitis

Summary: The study aimed to investigate the penetration of meropenem in cerebrospinal fluid (CSF) in patients with nosocomial ventriculitis and develop a nomograph to predict effective meropenem doses based on clinical parameters. The results showed that CSF penetration ratio was correlated with CSF protein levels, and a daily dose of 6 g/24 h of meropenem can achieve the target in patients with preserved renal function and low CSF protein levels.

Summary: Ventriculitis or post-neurosurgical meningitis or healthcare-associated ventriculitis and meningitis (VM) is a severe infection that complicates central nervous system operations or is related to the use of neurosurgical devices or drainage catheters. Diagnosis can be challenging, but novel biomarkers and molecular microbiology can expedite pathogen identification. Antibiotic treatment can be difficult due to poor penetration to the infection site. Intraventricular antibiotic therapy may help in cases of therapeutic failure or difficult-to-treat bacteria, but can cause complications. Regular removal of catheters or devices can prevent infections. New neurosurgical treatment modalities include debris evacuation and ventricle irrigation. Prevention measures include perioperative antibiotics, antimicrobial impregnated catheters, and standardized protocols for catheter insertion and manipulation.

Clinical Pharmacokinetics of Fosfomycin after Continuous Infusion Compared with Intermittent Infusion: a Randomized Crossover Study in Healthy Volunteers

Fosfomycin in continuous or prolonged infusion for systemic bacterial infections: a systematic review of its dosing regimen proposal from in vitro, in vivo and clinical studies

Summary: Intravenous fosfomycin is a promising option for treating systemic infections caused by multidrug-resistant bacteria. Studies have shown its activity against both Gram-negative and Gram-positive bacteria, with the need for coadministration of another active antibiotic to prevent the emergence of resistant strains. A therapeutic regimen including a loading dose of FOS 8 g followed by a daily dose of 16 g or 24 g continuous infusion may be the best approach for patients with normal renal function. Further research is needed on dosing regimens in patients with renal insufficiency and the superiority of continuous or prolonged infusion over intermittent infusion in clinical settings.

Meropenem concentrations in brain tissue of neurointensive care patients exceed CSF levels
